Growing evidence suggests that the Arg16Arg genotype of the beta-2 adrenergic receptor gene may be associatedwithadverseeffectsofbeta-agonisttherapy.Wesoughttoexaminetheassociationofbeta-agonistuseandthe Arg16Gly polymorphism with lung function and mortality among participants in the Atherosclerosis Risk in Communities study.MethodologyandPrincipalFindings.WegenotypedstudyparticipantsandanalyzedtheassociationoftheArg16Gly polymorphismandbeta-agonistusewithlungfunctionatbaselineandclinicalexaminationthreeyearslaterandwithall-cause mortality during 10years of follow-up. Lung function was characterized by percent-predicted forced expiratory volume in 1second.Associationswereexaminedseparatelyforblacksandwhites.Blackbeta-agonistuserswiththeArg/Arggenotype hadbetterlungfunctionatbaselineandatthesecondclinicalvisitthanthosewithArg/GlyandGly/Glygenotypes.Adjusted meanpercent-predictedFEV1was21%higherinArg/ArgsubjectscomparedtoGly/Glyatbaseline(p=0.01)and20%higher thanGly/Glyatvisit2(p=0.01).Arg/Glysubjectshadadjustedpercent-predictedFEV117%lowerthanArg/Argatbaselinebut weresimilartoArg/Argsubjectsat visit2.Althoughblackbeta-agonist userswiththeArg/Arg genotypeappeared tohave better crude survival rates, the association between genotype and all-cause
