BreastCancerIncidenceinBlackandWhiteWomen StratifiedbyEstrogenandProgesteroneReceptor Statuses MichaelX.Gleason,TengizMdzinarishvili,SimonSherman* EppleyCancerInstitute,UniversityofNebraskaMedicalCenter,Omaha,Nebraska,UnitedStatesofAmerica Abstract Background:Thereisincreasingevidencethatbreastcancerisaheterogeneousdiseasepresentedbydifferentphenotypes andthatwhitewomenhaveahigherbreastcancerincidencerate,whereasblackwomenhaveahighermortalityrate.Itis alsowellknownthatwhitewomenhavelowerincidenceratesthanblackwomenuntilapproximatelyage40,whenrate curvescrossoverandwhitewomenhavehigherrates.Thegoalofthisstudywastovalidatetheriskofwhiteandblack womentobreastcancerphenotypes,stratifiedbystatusesoftheestrogen(ER)andprogesterone(PR)receptors. Methodology/PrincipalFindings:SEER17datawerefractionedbyreceptorstatusinto[ER+,PR+],[ER2,PR2],[ER+,PR2], and [ER2, PR+] phenotypes. It was shown that in black women compared to white women, cumulative age-specific incidenceratesare:(i)smallerforthe[ER+,PR+]phenotype;(ii)largerforthe[ER2,PR2]and[ER2,PR+]phenotypes;and (iii)almostequalforthe[ER+,PR2]phenotype.Clemmesen’sHook,anundulationuniquetowomen’sbreastcancerage- specificincidenceratecurves,isshownheretoexistinbothracesonlyforthe[ER+,PR+]phenotype.Itwasalsoshownthat forallphenotypes,ratecurveshaveadditionalundulationsandthatage-specificincidenceratesarenearlyproportionalin allageintervals. Conclusions/Significance:Forblackandwhitewomen,riskforthe[ER+,PR+],[ER2,PR2]and[ER2,PR+]phenotypesare racedependent,whileriskforthe[ER+,PR2]phenotypeisalmostindependentofrace.Theprocessesofcarcinogenesisin aging,leadingtothedevelopmentofeachoftheconsideredbreastcancerphenotypes,aresimilarintheseracialgroups. Undulationsexhibitedonthecurvesofage-specificincidenceratesoftheconsideredbreastcancerphenotypespointtothe presenceofseveralsubtypes(tobedetermined)ofeachofthesephenotypes.
