Investigating its prevalence and trendsisessentialtoprevention. Methods:ToevaluatetheeffectivenessofHBVvaccinationunderthe1992InterventionProgramforinfantsandpredicted HBVprevalencetrendsunderthe2011Programforallages.Weconductedacommunity-basedinvestigationof761,544 residents of 12 counties in Zhejiang Province selected according to their location, population density, and economic development. TheHBV prevalence trendswere predictedby atime-shifting approach. HBV surfaceantigen (HBsAg) and alanineaminotransferase(ALT)weredetermined. Results: Of the 761,544 persons screened for HBsAg, 54,132 were positive (adjusted carrier rate 6.13%); 9,455 had both elevatedALTandapositiveHBsAgtest(standardizedrate1.18%).ThestandardizedHBsAgcarrierrateforpersonsaged#20 yearswas1.51%.KeyfactorsinfluencingHBVinfectionweresex,age,familyhistory,drinking,smoking,employmentasa migrantworker,andoccupation.Withthevaccinationprogramimplementedin2011,wepredictthatby2020,theHBsAg carrierratewillbe5.27%andthatforindividualsaged#34yearswillreachthe2%upperlimitoflowprevalenceaccording totheWHOcriteria,withastandardizedrateof1.86%. Conclusions:ThenationalHBVvaccinationprogramforinfantsimplementedin1992hasgreatlyreducedtheprevalenceof HBVinfection.The2011programislikelytoreduceHBVinfec
