Abstract
Background: Effect of fish oil supplementation on flow-mediated dilation, an index of endothelial function in humans,
remains controversial. We performed a meta-analysis to determine whether fish oil supplementation could improve
endothelialfunction.
Methods:HumaninterventionstudieswereidentifiedbysystematicsearchesofMedline,Embase,Cochrane’slibraryand
references of related reviews and studies. A random-effect model was applied to estimate the pooled results. Meta-
regressionandsubgroupanalyseswereperformedtoevaluatetheimpactofstudycharacteristicsontheeffectoffishoil
supplementationonflow-mediateddilation.
Results:Atotalofsixteenrecordswith1,385subjectswerereviewed.Theresultsofthepooledanalysisshowedthatfishoil
supplementationsignificantlyimprovedflow-mediateddilation(weighedmeandifference:1.49%,95%confidenceinterval
0.48% to 2.50%, p=0.004). Meta-regression and subgroup analysis suggested that the quality of included studies were
inverselyrelatedtotheoveralleffect(regressioncoefficient =21.60,p=0.04),andthesignificanceoftheeffectwasmainly
drivenbythestudieswithrelativelypoorquality.Sensitivityanalysisincludingonlydouble-blind,placebo-controlledstudies
indicatedfishoilsupplementationhasnosignificanteffectonendothelialfunction(weighedmeandifference:0.54%,95%
confidenceinterval20.25%to1.33%,p=0.18).Besides,normoglycemicsubjectsorparticipantswithlowerdiastolicblood
pressureseemedtobeassociatedwithremarkableimprovementofendothelialfunctionafterfishoilsupplementation.
Conclusions:Althoughcurrentevidencesuggestedapossibleroleoffishoilinimprovingendothelialfunction,large-scale
andhigh-qualityclinicaltrialsareneededtoevaluatetheseeffectsbeforewecancometoadefiniteconclusion.
