EffectsoftheAMPAAntagonistZK200775onVisual Function:ARandomizedControlledTrial RichardBergholz1*,ThomasStaks2,KlausRu¨ther1 1DepartmentofOphthalmology,Charite′ Universita¨tsmedizin Berlin,CampusVirchow-Klinikum,Berlin,Germany,2BayerScheringPharmaAG,Berlin,Germany Abstract Background:ZK200775isanantagonistatthea-Amino-3-hydroxy-5-methyl-4-isoxazolepropionate(AMPA)receptorand hadearnedattentionasapossibleneuroprotectiveagentincerebralischemia.ProbandsreceivingtheagentwithinphaseI trialsreportedonanalterationofvisualperception.Inthistrial,theeffectsofZK200775onthevisualsystemwereanalyzed indetail. Methodology: In a randomised controlled trial we examined eyes and vision before and after the intravenous administrationoftwodifferentdosesofZK200775andplacebo.Therewere3groupsof6probandseach:Group1recieved 0.03mg/kg/h,group20.75mg/kg/hofZK200775,thecontrolgroupreceived0.9%sodiumchloridesolution.Probands werehealthymalesagedbetween57and69years.Thefollowingmethodswereapplied:clinicalexamination,visualacuity, ophthalmoscopy,colourvision,rodabsolutethreshold,centralvisualfield,pattern-reversalvisualevokedpotentials(pVEP), ON-OFFandfull-fieldelectroretinogram(ERG). PrincipalFindings:NoeffectofZK200775wasseenoneyepositionormotility,stereopsis,pupillaryfunctionorcentral visual fieldtesting. Visualacuity anddark visiondeteriorated significantly inboth treatedgroups. Colorvisionwas most remarkablyimpaired.Thedark-adaptedERGrevealedareductionofoscillatorypotentials(OP)andpartlyofthea-andb- wave, furthermore an alteration of b-wave morphology and an insignificantly elevated b/a-ratio. Cone-ERG modalities showeddecreasedamplitudesanddelayedimplicittimes.IntheON-OFFERGtheON-answeramplitudesincreasedwhereas thepeaktimesoftheOFF-answerwerereduced.ThepatternVEPexhibitedloweramplitudesandprolongedpeaktimes. Conclusions:TheAMPAreceptorblockadeledtoastrongimpairmentoftypicalOFF-pathwayfunctionslikecolorvision andtheconeERG.OntheotherhandtheON-pathwayasmeasuredbydarkvisionandthescotopicE
