Methods:Aprospectivestudycomparedabaselineperiodwithaninterventionperiod.Duringtheintervention period,anICUhospitalpharmacistreviewedmedicationordersforpatientsadmittedtotheICU,notedissues relatedtoprescribing,formulatedrecommendationsanddiscussedthoseduringpatientreviewmeetingswiththe attendingICUphysicians.Prescribingissueswerescoredasprescribingerrorswhenconsensuswasreached betweentheICUhospitalpharmacistandICUphysicians. Results:Duringthe8.5-monthstudyperiod,medicationordersfor1,173patientswerereviewed.TheICUhospital pharmacistmadeatotalof659recommendations.Duringtheinterventionperiod,therateofconsensusbetween theICUhospitalpharmacistandICUphysicianswas74%.Theincidenceofprescribingerrorsduringthe interventionperiodwassignificantlylowerthanduringthebaselineperiod:62.5per1,000monitoredpatient-days versus190.5per1,000monitoredpatient-days,respectively(P0.001).PreventableADEs(patientharm,National CoordinatingCouncilforMedicationErrorReportingandPreventionseveritycategoriesEandF)werereduced from4.0per1,000monitoredpatient-d
