EndogenousH2ShasbeenproposedtobeinvolvedinMOFsinceplasmaH2Sconcentrationappearstoincreasein varioustypesofshocksandtopredictmortality.WetestedthehypothesisthatH2Sincreasesduringhemorrhagic shockassociatedwithO2deficit,andthatenhancingH2Soxidationbyhydroxocobalamincouldreduce inflammation,O2deficitormortality. Methods:Weusedaurethaneanesthetizedratmodel,where25ml/kgofbloodwaswithdrawnover30minutes. O2deficit,lacticacid,tumornecrosisfactor(TNF)-alphaandH2Splasmaconcentrations(Siegelmethod)were measuredbeforeandafterthebleedingprotocolincontrolanimalsandanimalsthatreceived140mg/kgof hydroxocobalamin.TheabilitytooxidizeexogenousH2Softheplasmaandsupernatantsofthekidneyandheart homogenateswasdeterminedinvitro. Results:Wefoundthatwithdrawing25ml/kgofbloodledtoanaverageoxygendeficitof122±23ml/kg.This O2deficitwascorrelatedwithanincreaseinthebloodlacticacidconcentrationandmortality.However,thelow levelofabsorbanceoftheplasmaat670nm(A670),afteraddingN,N-Dimethyl-p-phenylenediamine,thatis,the methodusedforH2Sdeterminationinpreviousstudies,didnotreflectthepresenceofH2S,butwasamarkerof plasmaturbidity.TherewasnodifferenceinplasmaticA670 beforeandafterthebleedingprotocol,despitethe largeoxygendeficit.Theplasmasampledattheendofbleedingmaintainedaverylargeabilitytooxidize exogenousH2S(highμM),asdidthehomogenatesofheartsandkidneysharvestedjustafterdeath. HydroxocobalaminconcentrationsincreasedinthebloodintheμMrangeinthevitaminB12group,and enhancedtheabilityofplasmaandkidneystooxidizeH2S.Yet,thesurvivalrate,O2deficit,H2 Splasma concentration,bloodlacticacidandTNF-alphalevelswerenotdifferentfromthecontrolgroup. Conclusions:InthepresenceofalargeO2deficit,H2Sdidnotincreaseinthebloodinaratmodelofuntreated hemorrhagicshock.Hy
