PerfusionImaginginPusherSyndrometoInvestigatethe NeuralSubstratesInvolvedinControllingUprightBody Position LucaFrancescoTicini1,UweKlose2,ThomasNa¨gele2,Hans-OttoKarnath1* 1SectionofNeuropsychology,CenterofNeurology,Hertie-InstituteforClinicalBrainResearch,UniversityofTu¨bingen,Tu¨bingen,Germany,2DepartmentofDiagnostic andInterventionalNeuroradiology,UniversityHospitalofTu¨bingen,Tu¨bingen,Germany Abstract Braindamagemayinduceadysfunctionofuprightbodypositiontermed‘‘pushersyndrome’’.Patientswithsuchdisorder sufferfromanalterationoftheirsenseofbodyverticality.Theyexperiencetheirbodyasorienteduprightwhenactually tiltednearly20degreestotheipsilesionalside.Pushersyndrometypicallyisassociatedwithposteriorthalamicstroke;less frequentlywithextra-thalamiclesions.Thisarguedforafundamentalroleofthesestructuresinourcontrolofuprightbody posture.Hereweinvestigatedwhethersuchpatientsmayshowadditionalfunctionalormetabolicabnormalitiesoutside theareasofbrainlesion.Weinvestigated19strokepatientswiththalamicorwithextra-thalamiclesionsshowingversusnot showingmisperceptionofbodyorientation.Wemeasuredfluid-attenuatedinversion-recovery(FLAIR)imaging,diffusion- weightedimaging (DWI), andperfusion-weighted imaging(PWI).This allowedus todeterminethestructural damage as well as to identify the malperfused but structural intact tissue. Pusher patients with thalamic lesions did not show dysfunctional brain areas in addition to the ones found to be structurally damaged. In the pusher patients with extra- thalamic lesions, the thalamus was neither structurally damaged nor malperfused. Rather, these patients showed small regionsofabnormalperfusioninthestructurallyintactinferiorfrontalgyrus,middletemporalgyrus,inferiorparietallobule, andparietalwhitematter.Theresultsindicatethattheseextra-thalamicbrainareascontributetothenetworkcontrolling uprightbodyposture.Thedataalsosuggestthatdamageoftheneuraltissueintheposteriorthalamusitselfratherthan additional malperfusion in distant cortical areas is
