Methods:Atotalof1229HIV-infectedpregnantwomenwereenrolled(2001–2004)inLusaka,Zambiaandfollowed topregnancyoutcome.Live-borninfantsweretestedforHIVbyPCRatbirth,1weekand5weeks.Obstetricand neonataldatawerecollectedafterdeliveryandtheratesofneonatal(28days)andearlymortality(70days)were describedusingKaplan-Meiermethods. Results:Theratioofmiscarriageandstillbirthper100live-birthswere3.1and2.6,respectively.Highermaternal plasmaviralload(adjustedoddsratio[AOR]foreachlog10increaseinHIVRNAcopies/ml=1.90;95%confidence interval[CI]1.10–3.27)andbeingsymptomaticwereassociatedwithanincreasedriskofstillbirth(AOR=3.19;95% CI1.46–6.97),anddecreasingmaternalCD4countby100cells/mm3withanincreasedriskofmiscarriage(OR=1.25; 95%CI1.02–1.54).Theneonatalmortalityratewas4.3per100increasingto6.3by70days.IntrauterineHIV infectionwasnotassociatedwithneonatalmoralitybutbecameassociatedwithmortalitythrough70days (adjustedhazardratio=2.76;95%CI1.25–6.08).Lowbirthweightandcessationofbreastfeedingweresignificant riskfactorsforbothneonatalandearlymortalityindependentofinfantHIVinfection. Conclusions:MoreadvancedmaternalHIVdiseasewasassociatedwithadversepregnancyoutcomes.Excess neonatalmortalityinHIV-infectedwomenwasnotprimarilyexplainedbyinfantHIVinfectionbutwasstrongly associatedwithlowbirthweightandprematurity.IntrauterineHIVinfectioncontributedtomortalityasearlyas 70daysofinfantage.Interventionstoimprovepregnancyout
