Baandrupetal.BMCPsychiatry2011,11:160 /1471-244X/11/160 STUDY PROTOCOL OpenAccess Prolonged-releasemelatoninversusplacebofor benzodiazepinediscontinuationinpatientswith schizophrenia:arandomizedclinicaltrial-the SMARTtrialprotocol LoneBaandrup1*,BirgitteFagerlund1,PoulJennum2,HenrikLublin1,JaneLHansen3,PerWinkel3,ChristianGluud3, BobOranje1andBirteYGlenthoj1 Abstract Background:Treatmentofschizophreniafrequentlyincludesprolongedbenzodiazepineadministrationdespitea lackofevidenceofitsuse.Itisoftendifficulttodiscontinuebenzodiazepinesbecauseofthedevelopmentof dependence.Weaimtoassessifmelatonincanfacilitatethewithdrawalofprolongedbenzodiazepine administrationinpatientswithschizophrenia.Furthermore,weaimtoinvestigatetheassociationof benzodiazepinedosereductionwiththefollowingclinicallyimportantvariables:sleep,psychophysiology,cognition, socialfunction,andqualityoflife. Methods/Design:Randomized,blinded,two-armed,parallelsuperioritytrial.Weplantoinclude80consenting outpatientsdiagnosedwithschizophreniaorschizoaffectivedisorder,18-55yearsofage,treatedwithantipsychotic drug(s)andatleastonebenzodiazepinederivativeforthelastthreemonthsbeforeinclusion.Exclusioncriteria: currentlyundertreatmentforalcoholordrugabuse,aggressiveorviolentbehavior,knownmentalretardation, pervasivedevelopmentaldisorder,dementia,epilepsy,terminalillness,severecomorbidity,inabilitytounderstand Danish,allergytomelatonin,lactose,starch,gelatin,ortalc,hepaticimpairment,pregnancyornursing,orlackof informedconsent.Afterbeingrandomizedtoprolonged-releasemelatonin(Circadin?)2mgdailyormatching placebo,participantsarerequiredtoslowlytaperofftheirbenzodiazepinedose.Theprimaryoutcomemeasureis benzodiazepinedoseat6monthsfollow-up.Secondaryoutcomemeasuresincludesleep,psychophysiological,and neurocognitivemeasures.Dataarecollectedatbaselineandat6monthsfollow-upregardingmedicaltreatment, cognition,psychophysiology,sleep,laboratorytests,adverseevents,psychopathology,socialfunction,andquality oflife.Dataonmedicaltr
