Chassidimetal.FluidsandBarriersoftheCNS2013,10:9 /content/10/1/9 FLUIDS AND BARRIERS OF THE CNS REVIEW OpenAccess Quantitativeimagingassessmentofblood-brain barrierpermeabilityinhumans YoashChassidim1,2*,RonelVeksler1,2,SvetlanaLublinsky1,2,GabySPell3,4,AlonFriedman1andIlanShelef2 Abstract Theblood–brainbarrier(BBB)isafunctionalandstructuralbarrierseparatingtheintravascularandneuropil compartmentsofthebrain.Itcharacterizesthevascularbedandisessentialfornormalbrainfunctions.Dysfunction intheBBBpropertieshavebeendescribedinmostcommonneurologicaldisorders,suchasstroke,traumatic injuries,intracerebralhemorrhage,tumors,epilepsyandneurodegenerativedisorders.ItisnowobviousthattheBBB playsanimportantroleinnormalbrainactivity,stressingtheneedforapplicableimagingandassessmentmethods. Recentadvancementsinimagingtechniquesnowmakeitpossibletoestablishsensitiveandquantitativemethods fortheassessmentofBBBpermeability.However,mostoftheexistingtechniquesrequirecomplicatedand demandingdynamicscanningprotocolsthatareimpracticalandcannotbefulfilledinsomecases.Wereview existingmethodsfortheevaluationofBBBpermeability,focusingonquantitativemagneticresonance-based approachesanddiscusstheirdrawbacksandlimitations.Inlightofthoselimitationsweproposetwonew approachesforBBBassessmentwithlessdemandingimagingsequences:the“post-pre”andthe“lineardynamic” methods,bothallowsemi-quantitativepermeabilityassessmentandlocalizationofdysfunctionalBBBwithsimple/ partialdynamicimagingprotocolsandeasy-to-applyanalysisalgorithms.Wepresentpreliminaryresultsandshow anexamplewhichcomparesthesenewmethodswiththeexistingstandardassessmentmethod.Westrongly believethattheestablishmentofsuch“easytouse”andreliableimagingmethodsisessentialbeforeBBB assessmentcanbecomearoutineclinicaltool.Largeclinicaltrialsareawaitedtofullyunderstandthesignificance ofBBBpermeabilityasabiomarkerandtargetfortreatmentinneurologicaldisorders.
