Methods:Acase-controlstudywasperformedwith1,571arthritispatientsonlong-termlow-doseMTXtherapy. Resultsofliverbiopsywereanalyzedin41patientswithelevatedliverenzymes.Theexpressionofautoimmune markerswasalsoassessed.Thispopulationwascomparedwith41diseasecontrolsubjectsobtainedfromthe samedatabase,alsoonMTXbutwithoutelevatedliverenzymes,matchedforage,sexandrheumaticdisease. Results:Comparedwiththediseasecontrols,patientswithliverbiopsyshowedlowerdiseasedurationandlower MTXexposure,weeklyandcumulativedoses,reflectingshortertreatmentdurationduetoliverabnormalities.Liver biopsiesshowed17autoimmunehepatitis-like(AIH-like)lesions,13nonalcoholicsteatohepatitis-likelesions,seven limitedliverlesions,andtwoprimarybiliarycirrhoses.However,MTX-specificlesionswithdystrophicnucleiin hepatocyteswereseeninonlytwocases.Liverbiopsylesionswereassociatedwithautoimmunemarkers(P= 0.007);notably,AIH-likelesionswereassociatedwithrheumatoidarthritisandwiththepresenceoftheHLA-DR sharedepitope. Conclusions:MTX-specificliverlesionsarerarelyobservedinarthritispatientsunderlong-termMTXtherapyand elevatedliverenzymes.
