Findings:Underthesuspicionofherpeticeyedisease,weadministeredtopicalandsystemicanti-herpetic treatmentafterthesecondrepeatkeratoplasty.Thepostoperativecoursewasuneventful,andthecornealgraftis clear,untilrecently.ImmunohistochemistryandDNA-polymerasechainreactionwerenegativeforherpessimplex virus-1(HSV-1)inthehostcornea,buttheydetectedHSV-1inbothtransplantedcornealgrafts,therebysupporting ourclinicalhypothesisthatgraft-to-hostHSV-1infectionelicitedthischainreactionofcomplicationsinourpatient. Conclusion:Thisclinicalreportillustratesinauniquewaythedramaticimpactanunsuspectedherpeticinfection inthecornealgraftincasesofkeratoplastymayhaveandunderscoresthenecessityofsuspectingandadequately treatingthesedistinctcases.
