Methodology/PrincipalFindings:WestudiedrisksforBVacquisitioninaprospectivecohortstudyofwomen(age16–35 years)whoreportedsexwithotherwomen($1partner,prioryear).Womenwerefollowedforoneyearwithexaminations atquarterlyvisitsandforgenitalsymptomsatanytime.Species-specific 16SrRNAgenePCRsforBV-associated bacteria (BVAB) were applied to vaginal fluid obtained at enrollment. Sexual behaviors were ascertained by computer-assisted interview.Of335participants,239hadnoBVatbaseline;199wereseeninfollow-up(medianfollow-up355days,4.0visits/ subject).Fortywomenexperienced$1BVepisode.RisksforincidentBVwerepresentation#14dayssinceonsetofmenses (hazardratio(HR)2.3(95%CI,1.2–4.7),reportofnewsexpartnerwithBVhistory(HR3.63(1.1–11.9)),changeinvaginal discharge(HR2.6(1.3–5.2))anddetectionofanyofseveralBVABinvaginalfluidatenrollment,includingBVAB1(HR6.3 (1.4–28.1)),BVAB2(HR18.2(6.4–51.8)),BVAB3(HR12.6(2.7–58.4)),G.vaginalis(HR3.9(1.5–10.4)),Atopobiumvaginae(HR4.2 (1.9–9.3)), Leptotrichia spp (9.3 (3.0–24.4)), and Megasphaera-1 (HR 11.5(5.0–26.6)). Detection of Lactobacillus crispatus at enrollmentconferredreducedriskforsubsequentBV(HR0.18(0.08–0.4)).Detailedanalysisofbehavioraldatasuggesteda directdose-responserelationshipwithincreasingnumberofepisodesofreceptiveoral-vulvovaginalsex(HR1.02(95%CI, 1.00–1.04).
