MethodsandFindings:Weconductedapopulation-basedcase-controlstudyofconsecutivemaleyouthbetweenage16 and19yearshospitalizedforroadtrauma(cases)orappendicitis(controls)inOntario,Canadaover7years(April1,2002 throughMarch31,2009).Usinguniversalhealthcaredatabases,weidentifiedpriorpsychiatricdiagnosesforeachindividual duringthedecadebeforeadmission.Overall,atotalof3,421patientswereadmittedforroadtrauma(cases)and3,812for appendicitis(controls).Ahistoryofdisruptivebehaviordisorderswassignificantlymorefrequentamongtraumapatients thancontrols(767of3,421versus664of3,812),equaltoaone-thirdincreaseintherelativeriskofroadtrauma(oddsratio = 1.37,95%confidenceinterval1.22–1.54,p,0.001).Theriskwasevidentoverarangeofsettingsandafteradjustmentfor measuredconfounders(oddsratio1.38,95%confidenceinterval1.21–1.56,p,0.001).Theriskexplainedaboutone-in-20 crashes, was apparent years before the event, extended to those who died, and persisted among those involved as pedestrians. Conclusions:Disruptivebehaviordisordersexplainasignificantamountofroadtraumainteenagemaleyouth.Programs addressingsuchdisordersshouldbeconsideredtopreventinjuries.
