Methods:SuicidalideationwasrecordedusingthespecificsuicideitemoftheHamiltonDepressionRatingScale (HDRS)inacohortofsubjectsaged12-30years(N=494)attendingheadspaceservices. Results:Ofthe494youngpersonsassessed,32%(158/494)hadapositiveresponsetoanyleveloftheHDRS suicideitem,consistingof16%(77/494)reportingthatlifewasnotworthlivingandafurther16%(81/494) reportedthoughtsofdeathorsuicidalideation.Youngwomen(19%;94/494)weremorelikelytoreportany positiveresponseascomparedwithyoungmen(13%;64/494)[χ2(2,494)=13.6,p.01].Thosewith‘attenuated syndromes’reportedpositiveresponsesatratescomparabletothosewithmoreestablisheddisorders(35%vs.34%; χ2(1,347)=0.0,p=0.87).However,moreseriouslevelsofsuicidalideationweremorecommoninthosewith depressivedisordersorlaterstagesofillness.Inmultivariateanalyses,themajorpredictorsofthedegreeofsuicidal ideationwereincreasinglevelsofclinician-rateddepressivesymptoms(beta=0.595,p.001),general psychopathology(beta=0.198,p.01),andself-reporteddistress(beta=0.172,p.05). Conclusions:Feelingsthatlifeisnotworthliving,thoughtsofdeathorsuicidalideationarecommoninyoung peopleseekingmentalhealthcare.Theseat-riskcognitionsareevidentbeforemanyoftheseindividualsdevelop severeorpersistentmentaldisorders.Thoughtsofdeathorsuicidalideationmaywellneedtobeaprimary interventiontarget
