Methods:Sixty-oneHCCpatientswereincluded.Liverspecimenswereobtainedeitherbybiopsy(n=17)or surgically(resection27,transplantation17).ImmunostainingforAFP,Ki-67,SCCAandTUNELassaywereperformed. Results:SCCAstainingwasdetectedin83.6%ofspecimens.Astatisticalsignificantcorrelationwasfoundbetween negativeSCCAstainingandmortality(p=0.026)andahigherimmunostainingscoreforKi67(p=0.017).Positive SCCAstainingwasassociatedwithwellandmoderatedifferentiatedtumors(p=0.022).Usingmultiplelogistic regressionanalysis,Ki67andTUNELassaywerefoundtobesignificantindependentpredictorsofnegativeSCCA immunostaining.Theareaunderthereceiveroperatorcharacteristiccurvewas0.87.Kaplan-Meiersurvivalanalysis revealedasignificantdifferencebetweenthepatientgroupwithpositiveversusnegativeSCCAimmunostaining relatingtosurvivaltime(p=0.0106).CoxproportionalhazardregressionanalysisdemonstratedthatKi67 immunostainingandlivertransplantationorresectionwereindependentlyassociatedwithmortality. Conclusions:SCCAisoverexpressedinHCC.SCCAstatusisassociatedwithcellproliferation,apoptosisand survival.SCCAandKi67stainingcanpredictsurvival.Ourstudyresultssupportapotentialassociationofnegative SCCAexpressionwithothermarkersofpooroutcomeinHCC.MorestudiesareneededtoclarifytheroleofSCCA inHCCandexpandtheknowledgeoftheSCCAantigeninHCCpatients.
