Methods:InthisstudyweevaluatedinahumanE.colisepsiscohort,theroleofPRRincludingTLR’sandTrem-1. ExpressionofTrem-1,TLR2,TLR4,CD14andHLA-DRonbloodmonozytesandneutrophilswereexaminedusing flowcytometryfrom22patientswithE.colisepsisand6healthycontrols.LPSandLTAstimulatedTNFalpha,IL-10, IL-8andIL-6productionwasstudiedina24hwholebloodassay.FreecytokineserumconcentrationofTNFalpha, PCTandIP-10wereevaluated. Results:WefoundasignificanthigherexpressionofTrem-1andTLR-2onmonocytesandneutrophilsinpatients comparedtohealthyvolunteers.TLR2expression(p0.05)washigherandHLA-DRlower(p0.05)onmonocytes ofpatientswithseveresepsiscomparedtopatientswithsimplesepsis.Trem-1expressionwastendentiallyhigher (p=0,07)onmonocytesandloweronneutrophilsofpatientswithseveresepsis.Trem-1expressiononneutrophils wasassociatedwiththeIL-10(LPS:r=0,61,p0.02)andTNF-αinducibility(LPS:r=0,78,p0,002).Inaddition Trem-1expressiononneutrophilsshowsanegativecorrelationtotheserumlevelsofTNFalpha(r=?0,63;p 0,005),IP-10(r=?0,5;p0,035)andprocalcitonin(r=?0,59;p0,007). Conclusions:PatientswithE.colisepsisarecharacterizedbyanassociationofTrem-1expressiononblood neutrophilswithcytokineinducibility.TheTREM-1pathwayonneutrophilsmightplayaroleinproducingan adequateinflammatoryandbactericidalresponseinbacterialsepsis.
