Lietal.DiagnosticPathology2012,7:64 /content/7/1/64 CASE REPORT OpenAccess UnusualprimaryosseousHodgkinlymphomain ribwithassociatedsofttissuemass:acasereport andreviewofliterature YangLi1,Xiao-bingWang2,Xiao-yingTian3,BinLi1andZhiLi1* Abstract Hodgkinlymphoma(HL)typicallypresentsasnodallesionandmayinvolveextranodalsitesduringtheprogression ofthedisease.PrimaryosseousHLwithoutanylymphnodeassociationisextremelyrareandonlyafewsuchcases havebeendescribedintheliterature.WepresentacaseofunusualprimaryHLinriboccurringinamiddle-aged femalepatient.Computedtomography(CT)scanrevealedanosteolyticlesionwaslocatedattherightsecondrib andwasassociatedwithalargesofttissuemass.Therewasnoregionallymphnodeinvolvement.CTscanofneck andabdomenwasperformedandshowednopathologicfindings,particularlynolymphadenopathyand organomegalycouldbeobserved.Histologically,typicalbinucleatedReed-Sternberg(RS)cellsandlacunarcells werescatteredinthebackgroundofreactiveinflammationwithinfiltrationoflymphocytes,histiocytesand eosinophilicgranulocytes.Byimmunohistochemistry,RScellsandlacunarcellswerepositiveforCD15andCD30 withtypicalmembraneandparanucleardot-likestainingpattern.However,thesecellswerenegativefor Epstein-Barrvirusdetectionbyinsituhybridization.AdiagnosisofprimaryosseousHLwasmade.Thepatient receivedsystemicchemotherapyandlocalradiotherapy,andwasonregularfollow-upfor24months.Therewasno signofrecurrenceoftumorandlymphnodeorbonemarrowinvolvement.Becausethereisapossibilityof secondaryboneinvolvementbysystemicHL,stricthistologicalanalysisandthoroughradiographicexaminationare suggestedtobenecessaryforaccuratelydiagnosingthistumorwhenitpresentsasasolitarybonelesion.
