Methods:142consecutivepatientswithaninitialDSM-IV-TRdiagnosisofamajordepressiveepisodewere screenedforBPDusingtheChinesetranslationoftheMDQandfollowedupforoneyear.Thefinaldiagnosis, determinedbyaspecialcommitteeconsistingofthreetrainedseniorpsychiatrists,wasusedasa‘goldstandard’ andROCwasplottedtoevaluatetheperformanceoftheMDQ.Theoptimalcut-offwaschosenbymaximizingthe Younden’ sindex. Results:Ofthe142patients,122(85.9%)finishedtheoneyearfollow-up.Onthebasisofasemi-structuredclinical interview48.4%(59/122)receivedadiagnosisofunipolardepression(UPD),36.9%(45/122)BPDIIand14.8%(18/ 122)BPDI.Attheendoftheoneyearfollow-up,9movedfromUPDtoBPD,2fromBPDIItoUPD,1fromBPDIIto BPDI,theoverallrateofinitialmisdiagnosiswas16.4%.MDQshowedagoodaccuracyforBPD:theoptimalcut-off was4,withasensitivityof0.72andaspecificityof0.73.WhenBPDIIandBPDIwerecalculatedindependently,the optimalcut-offforBPDIIwas4,withasensitivityof0.70andaspecificityof0.73;whiletheoptimalcut-offforBPDI was5,withasensitivityof0.67andaspecificityof0.86. Conclusions:OurresultsshowthattheChineseversionofMDQisavalidtoolforscreeningBPDinagroupof patientswithcurrentdepressiveepisodeontheChinesemainland.
