Baumannetal.CriticalCare2011,15:R179 /content/15/4/R179 RESEARCH OpenAccess Fiberopticbronchoscopyinpatientswithacute hypoxemicrespiratoryfailurerequiring noninvasiveventilation-afeasibilitystudy HansJ?rgBaumann1,2?,HansKlose2?,MarcelSimon1,2,TarikGhadban1,StephanABraune1,JanKHennigs2and StefanKluge1* Abstract Introduction:Noninvasiveventilation(NIV)isastandardprocedureinselectedpatientswithacuterespiratory failure.Previousstudieshaveusednoninvasiveventilationtoensureadequategasexchangeduringfiberoptic bronchoscopyinspontaneouslybreathinghypoxemicpatients,thusavoidingendotrachealintubation.However,it isunknownwhetherbronchoscopycanbeperformedsafelyinpatientswithacutehypoxemicrespiratoryfailure alreadyinneedofNIVpriortothedecisionforbronchoscopy. Methods:Weprospectivelyinvestigated40consecutive,criticallyill,adultpatientswithacutehypoxemic respiratoryfailure(14women,26men,age61±15years,partialpressureforoxygen/fractionofinspiredoxygen (PaO2/FiO2)300undernoninvasiveventilation,SimplifiedAcutePhysiologyscores(SAPSII)47±9.9points).All patientsrequirednoninvasiveventilationpriortothedecisiontoperformbronchoscopy(median10.5h;range2.2 to114).Bloodgases,heartrate,bloodpressureandventilationweremonitoredbefore,duringandupto120 minutesafterbronchoscopy. Results:Bronchoscopycouldbecompletedinallpatientswithoutsubsequentcomplications.Oxygensaturation fellto90%intwopatients(5%),andthelowestvalueduringtheprocedurewas84%.ThemeanPaO2/FiO2ratio improvedfrom176±54atbaselineto240±130(P0.001)attheendofbronchoscopyand210±79after120 minutes.Thetransientmeanpartialpressureofcarbondioxideinthearterialblood(PaCO2)increasewas9.4±8.1 mmHg.Fourpatients(10%)requiredendotrachealintubationduringthefirsteighthoursaftertheprocedure. Bronchoalveolarlavageyieldeddiagnosticinformationin26of38(68%)patients. Conclusions:Incriticallyillpatientswithacutehypoxemicrespiratoryfailurerequiringnoninvasiveventilation, bronchoscopycanbeperformedwithanacceptablerisk.Sincethesepatientspersehaveahighlikelihoodof
