改良经皮内固定治疗新鲜肩锁关节脱位 　　【摘要】 目的 介绍改良闭合复位经皮穿钉内固定 治疗 肩锁关节脱位的治疗 方法 。方法 患者仰卧于手术台上，手法复位后用一枚Φ2mm的克氏钉通过进钉定位器穿入肩峰并固定肩锁关节，术后三角巾悬吊6周，3个月内避免重体力劳动，3个月后拔钉。结果 1例术后因钉尾顶破皮肤致钉尾部位感染而于4周内拔除克氏钉，1例3个月复查时克氏钉轻度折弯，但肩锁关节未见脱位，6～12个月随访，有1例肩锁关节出现半脱位，疗效按karlsson分级:A级11例，B级1例。结论 该方法简单易行，创伤小，疗效好，费用低，无切口感染及疤痕障碍等并发症。 　　关键词 肩锁关节 脱位 闭合复位 克氏钉 内固定 　 　　 　　 The method of improving restoration and a Knowles nail penetrating shoulderpeak via skin and fixing for treating acromioclavicular joint dislocation 　 　　【Abstract】 Objective Introduce the method of improving restoration and a Knowles nail penetrating shoulder peak via skin and fixing for treating acromioclavicular dislocation.Methods Patient lie supine on operation table.Taking a Knowles nail penetrates shoulder peak through positionerandfixacromioclavicular after restoration.The upˉper limb is suspended by triangle cloth for6weeks after operation and avoid heavy manual labor in3month.Pull out the nail3months later.Results Knowles nail tail position is infected in a case and ;pulled out it in4week.Knowles nail is curved in a case when examining in3month，half dislocation in a case when to follow up6～12month.Curaˉtive effect grade according to Karlsson:11cases of A grade，1case of B grade.Conclusion This method is easy，the wound is little，curative effect is good，it is cheap to treat the expenses，not infecting and scar obstacle etc. 　　Key words acromioclavicular joint dislocation restoration Knowles nail internal fixation 　　肩锁关节脱位的治疗方法很多，有关这方面报道是所有骨科疾病治疗中最多的 ［1］ ，各有优缺点，其中经皮穿钉内固定治疗肩锁关节脱位是一种久远的方法 ［2］ 。具有创伤小而且有效的特点，我院自2001年1～2004年3月共收治新鲜肩锁关节脱位12例，采用在闭合复位经皮穿钉内固定技术的基础上作一些改进的治疗方法，获得满意的效果。现报告如下。 　　1 临床资料 　　1.1 一般资料 本组共12例患者，其中男10例，女2例;年龄最大53岁，最小22岁;摩托车撞伤8例，跌伤4例;按Tossy分类Ⅱ型2例，型10例。除8例合并皮肤擦伤或破损外无其它合并症。所有病例受伤后1～24h内入院，入院后立即手术者5例，2～3天内手7例。 　　1.2 治疗方法 采用血肿内麻醉或肌间沟内麻醉，麻醉妥后患者仰卧于手术台上，背脊沟部位垫一个沙袋，使两侧肩部 自然 后伸，术者并可将两肩向后施压，以便于矫正锁骨外侧端的向后移位。助手站在伤者的受伤侧，用一手的第2～5指将锁骨向前下方牵拉，用另一手握住患者伤侧上肢将其向上推使肩锁关节复位。术者用手触摸肩锁关节表面，如果阶梯感消失，关节表面平坦，说明复位成功，或者用C型臂X光机检查肩锁关节是否复位。复位后助手维持上述体位，术野
