结直肠肛管癌3197例流行病学分析 作者：张小龙 高枫 陈利生 刘小健 唐卫中 【摘要】 目的 探讨近年来结直肠肛管癌的发病情况及其变迁。 方法 收集1993～2007年间收治的结直肠肛管癌患者发病资料，进行分组和统计比较。结果 共收治3197例患者，发病中位年龄为男性58，女性54岁。按发病时间先后，每5年为1组，患者例数明显增加，中位发病年龄由53岁上升至57岁；男女患者的比例为1.461，各组间无明显差异；直肠癌比例由64.45% 下降至54.99%，左、右半结肠比例分别由18.38%和14.51%上升至23.81%和19.64%。结论 结直肠癌发病年龄女性较男性提前。15年来年轻患者比例下降，高龄患者增加，发病年龄后移。男性患者居多，性别比例变化不明显。发病部位以直肠为主，但所占比例明显下降。 【关键词】 流行病学；结直肠肛管癌； 分析 Epidemiological analysis in 3197 colorectal-anal cancer patients ［Abstract］ Objective To explore the epidemical distribution characteristics of colorectal-anal cancer and its’ diversify in recent years.Methods The data of colorectal-anal cancer patients treated in our division from 1993 to 2007 was collected and analyzed. Results Data of 3197 patients was analyzed. The median peak age was 58 in male and 54 in female. The number of patients increased from 751 in the first 5 years to 1344 in the third 5 years. The median peak age also increased from 53 to 57. The ratio between male and female was 1.46 and hadn’t changed in the three 5 years. The ratio of rectal cancer decreased from 64.45% to 54.49%. At the same time, the ratio of left colon cancer increased from 18.38% to 23.81% and right colon cancer increased from 14.51% to 19.64%. Conclusion The morbility of colorectal cancer increased in recent years. The age of onset in female is earlier than male. The ratio of young patients decreased and patients of eld increased which caused the increasing of the median peak age. Male patients is more than female and the ratio between them was invariably. Rectal cancer is more than colon cancer, but the superiority became more and more smaller. ［Key words］ Epidemiology；Colorectal-anal cancer；Analysis 结直肠癌在世界范围内均属高发，一些欧美国家的发病率排在恶性肿瘤第2、3位。与其比较， 中国 属于低发地区，但近年来发病率逐年上升。流行病学调查发现，结直肠癌在发病年龄、部位和性别比等方面近年来出现了较明显的变化，国内的 研究 也有类似发现。本科室自1993年以来共收治结直肠肛管癌3 197例，本文对其发病情况进行分析，以了解15年来结直肠癌的发病及变迁情况。 1 临床资料 1993年至2007年9月共收治结直肠肛管癌患者3197例，其中男性1896例，女性1301例；年龄11～89岁；发病部位肛管57例，直肠1833例，左半结肠694例（其中乙状结肠550例，降结肠103例，结肠脾曲
