湖南省老年人就医难现状及对策研究 　　[摘要] 目的 探讨湖南省老年人就医难现状及不同类别老年人“就医难”的差异及影响因素。 方法 根据经济社会发展水平不同，采用多阶段分层整群抽样法，抽取湖南省株洲市、衡阳市、湘西自治州3个地区作为样本，对调查地区60岁及以上老年人进行问卷调查。共发放问卷600份，回收有效问卷589份。所有数据通过SPSS 22.0进行录入处理，对统计分析结果进行多元有序Logistic回归分析。 结果 老年人年龄越大，就医方便性越差；农村老年人就医方便性（85.3%）显著低于城市（94.7%）。在95%的置信水平下，老年人就医满意度受到年龄和文化程度两个因素的显著影响。“认为自己病得不严重”（41.2%）和“去医院太花钱”（28.2%）是湖南省老年人“就医难”的关键影响因素。 结论 医保报销政策适当向老年群体倾斜，关注独居老人健康和就医，推进医养一体化，以缓解老年人“就医难”的问题 [关键词] 老年人；就医难；可及性；看病贵；医养结合 [中图分类号] R161.7 [文献标识码] A [文章编号] 1673-7210（2016）09（c）-0075-04 [Abstract] Objective To find the current situation of difficulty in seeking medical treatment and the differences， influencing factors for various types of elderly people. Methods Zhuzhou City， Hengyang City and Xiangxi Autonomous Prefecture were selected as samples under the multistage stratified cluster sampling method according to the different level of economic and social development. 600 questionnaires were issued to the aged over 60 and 589 questionnaires were collected. All the data were input and processed by SPSS 22.0. The statistic analysis result was researched by multivariate ordered Logistic regression. Results The convenience of seeking medical treatment for elderly people was lower than for young people. The convenience of rural elderly people （85.3%） was significantly lower than urban ones （94.7%）. Under the confidence level of 95%， elderly people’s satisfaction on medical services was influenced significantly by their age and degree of education. Thinking their disease not serious （41.2%） and high medical expenses （28.2%） were found as key influencing factors which leaded to difficulty in seeking medical treatment for elderly. Conclusion Preferential medical reimbursement policies for elderly people， paying more attention to elderly people living alone and carrying out medical providing integration can improve the situation of difficulty in seeking medical treatment for elderly people.
