經核實決議副本修訂 / 終止電話理財及自動櫃員機服務 Certified Copy of Resolutions Amendment / Termination for Phone Banking and ATM Services 致 : 恒生銀行有限公司 日期(日/月/年) To : Hang Seng Bank Limited Date(DD/MM/YY) 基本戶口號碼 Primary Account No. 公司 / 商號 / 組織名稱 ( 「客戶」) Name of Company / Firm / Organisation (the “Customer”) 註冊辦公室 / 營業地址 Registered Office / Business Address 鑑於客戶擬修訂及/或終止與恒生銀行有限公司（「貴行」）就使用電話理財及/或自動櫃員機服務（「服務」）之安排。 WHEREAS the Customer desires to amend and/or terminate the arrangement with Hang Seng Bank Limited (the“Bank”) for using the phone banking and/or ATM services (the “Services”). 謹此議決通過 NOW THEREFORE BE IT RESOLVED THAT 1. 客戶指示及授權貴行按隨本決議附呈之修訂/終止表格（「修訂/終止表格」）所列，修訂及/或終止使用服務之安排； the Customer instructs and authorises the Bank to amend and/or terminate the arrangement for using the Services in the manner specified in the amendment/termination form attached to these Resolutions (the “Amendment/Termination Form”); 2. 於會議上呈案所示之修訂/終止表格將會及經已獲得通過接納，並符合客戶之商業利益； the Amendment/Termination Form in the form tabled at the Meeting shall be and is approved and accepted and it is in the commercial interest of the Customer to do so; 3. 此等決議須向貴行呈報及維持有效，直至修訂決議經客戶之董事會或管理組織通過（視屬何情況而定），及貴行收到經會議 主席核實之副本 ；及 these Resolutions shall be communicated to the Bank and remain in force until an amending resolution shall be passed by the board of directors or the governing body (as the case may be) of the Customer and a copy certified by the chairman of the meeting shall have been received by the Bank; and 4. 除於本決議所列明外，現時就使用服務之安排，將維持完全有效及作用。 except to the extent specified in these Resolutions, the existing arrangement for using the Services shall remain in full force and effect.
