研究胃大部切除术与单纯穿孔修补术治疗胃十二指肠溃疡穿孔的临床疗效 　　摘要：目的 探讨研究在治疗胃十二指肠溃疡穿孔中胃大部切除术和单纯穿孔修补术的临床疗效。方法 选取我院在2010年5月～2015年5月收治的200例胃十二指肠溃疡穿孔患者进行研究，采用随机数字法将患者分为对照组100例胃大部切除术，观察组100例单纯穿孔修补术。结果 在手术时间、肛门排气时间、住院时间方面，术中出血量方面，观察组明显短于对照组（P?0.05），在术中出血量方面，观察组明显少于对照组（P?0.05），在术后感染和并发症等方面，观察组的情况也明显优于对照组（P?0.05），在手术时间、肛门排气时间、住院时间方面，腹腔镜手术患者所用的时间也明显短于开腹手术患者（P?0.05），腹腔镜手术患者的术后感染率以及并发症也明显低于开腹手术患者（P?0.05）。结论 在临床疗效方面，胃大部切除术与单纯穿孔修补术相近，但腹腔镜手术的创伤小、操作简单，在微创方面具有显著的优势，更适合用于手术治疗胃十二指肠溃疡穿孔 关键词：胃大部切除术；单纯穿孔修补术；胃十二指肠溃疡穿孔；临床疗效 Abstract：Objective To study the research most of gastric resection in the treatment of gastroduodenal ulcer perforation and the clinical curative effect of simple perforation repair.Methods Our hospital in May 2010 to May 2015 treated 200 cases of patients with gastroduodenal ulcer perforation are studied，according to the different surgical methods are divided into most of the control group，100 cases of gastric resection，observing group，100 cases of simple perforation repair.Results The operation time，length of hospital stay，anal exhaust time，intraoperative blood loss，observation group is shorter than the control group obviously（P?0.05），in terms of intraoperative blood loss，obviously lower in observation group than the control group（P?0.05），in terms of postoperative complications such as infections and，observation group is obviously better than the control group（P?0.05），the operation time，length of hospital stay，anal exhaust time，time is also used in the patients with laparoscopic surgery patients significantly shorter than laparotomy（P?0.05），postoperative infection and complications of laparoscopic surgery patients is significantly lower than laparotomy patients（P?0.05）.Conclusion in the aspect of clinical curative effect，the stomach was close to pure perforation repair，but small trauma，simple operation，laparoscopic surgery has significant advantage in minimally invasive，more suitable for the surgical treatment of gastroduodenal ulcer perforation.
