我院静脉用药调配中心不合理用药干预效果分析 　　[摘要] 目的 通过实施不合理用药干预，促进临床合理用药，确保用药安全。 方法 采用回顾性分析法，收集干预前我院静脉用药调配中心（PIVAS）2015年的不合理用药和干预后2016年的不合理用药，对不合理用药干预效果进行统计分析。 结果 2015年共??核医嘱474523份，其中不合理用药3102份，主要包括医嘱录入错误、用法用量不合理、溶媒不合理、给药途径、配伍禁忌等方面的问题。干预后，不合理用药由0.65%下降为0.20%，不合理用药修改率由99.3%上升为99.9%。 结论 PIVAS药师通过干预不合理用药能有效地预防与减少因不合理用药而引发的不良事件，促进合理用药 [关键词] 静脉用药调配中心；不合理用药；干预；药师 [中图分类号] R952 [文献标识码] B [文章编号] 1673-9701（2017）10-0105-04 Effect analysis of irrational drug intervention of pharmacy intravenous admixture service in our hospital WANG Xiaojian DENG Xue’e Department of Pharmacy， Beijing Friendship Hospital Affiliated to Capital Medical University， Beijing 100050，China [Abstract] Objective To promote the clinical rational use of drugs and ensure the safety of medication through the implementation of irrational drug intervention. Methods The unreasonable medications before intervention in 2015 and after intervention in 2016 in pharmacy intravenous admixture service were collected by retrospective analysis method. And the intervention effect of irrational drug use was statistically analyzed. Results A total of 474523 copies of doctor’s advice were audited in 2015， of which there were 3102 cases of irrational drugs， mainly including the doctor input error， unreasonable usage and dosage， unreasonable solvent， the route of administration， incompatibility. After intervention， the irrational medication decreased from 0.65% to 0.20%， and unreasonable drug modification rate increased from 99.3% to 99.9%. Conclusion PIVAS pharmacists can effectively prevent and reduce the adverse events caused by unreasonable drug use and promote the rational use of drugs by interfering with unreasonable drugs.
