摘 要 MRI 征象在四肢良恶性软组织肿块鉴别诊断的价值 研究生：褚华鲁 导师：李白艳 教授 摘 要 目的：探讨四肢软组织肿瘤良恶性的核磁共振成像(MRI)征象，进一步提高 MRI 诊断良恶性四肢软组织肿瘤的价值。方法：对经病理证实的 42 例四肢软组织肿块（良 性 29 例，恶性 13 例）的 MRI 征象（范围、边缘、信号、瘤周水肿、侵袭性）进行 分析，分析其对良恶性肿瘤鉴别诊断中的价值，统计学方法用四格表确切概率法。 结果：（1）MRI 对 42 例四肢软组织肿瘤的恶性诊断准确率为 92.31%，良性准确率 为 93.10% 。（2 ）在 MRI 征象中，侵袭性及瘤周水肿对恶性肿瘤的诊断有统计学意 义（P ＜0.05 ），其余3 项（范围、边缘、信号）无统计学意义（P ＞0.O5 ）。结论： 在四肢软组织肿瘤的定性诊断中，侵袭性及瘤周水肿 MRI 征象对恶性肿块的诊断具 有较大的可靠性。 关键词： 四肢软组织肿瘤；核磁共振成像 —1— 新疆医科大学医学硕士学位论文 The Value of MRI Imaging In Differentiating Benign of Soft Tissue in Extremities Abstract Objective：To investigate the MRI features of benign and malignant soft-tissue tumors and so as to improve the ability of diagnosing them with MRI. Methods: MRI signs (invasion edema .disseminated extension,margin indenfinite and mixed signal intensity of tumors) of 42 cases tumor of soft tissue (benign 29,malignant 13) confirmed pathology were analyzed .The value of MRI signs on diagnostic tumor of soft tissue was evaluated.The method of statistics was exact probabilities in fourfold. Results ：(1)The qualitative diagnostic accurate rate for benign was 93.10% and malignant was 92.31%. (2)The finding of invasion and edema around the masses in diagnosis of malignant tumors had a remarkable statistical significance among five signs （P ＜0.01 ），but another three MRI signs (disseminated extension,margin indenfinite and mixed signal intensity of tumors) had no statistical significance （P ＞0.O5 ）Conclusion：MRI imaging is very helpful for diagnosis of the benign and malignant soft tissue tumors.
