单纯椎弓根内固定及椎弓根内固定结合经椎弓根植骨治疗胸腰段爆裂骨折的临床疗效分析.pdfVIP

提 要 目的：分析比较单纯椎弓根钉棒系统内固定与椎弓根钉棒系统内固定结合经椎弓 根椎体内植骨治疗胸腰段（T11～L2）爆裂骨折的临床疗效，为临床治疗提供参考。 方法：分析山东中医药大学附属医院脊柱骨科2009年2月至 2011年2月收治的 60 例胸腰段爆裂骨折患者，分为两组：不植骨组和植骨组。采用单纯椎弓根钉棒系 统内固定（不植骨组）和椎弓根钉棒系统内固定结合经椎弓根椎体内植骨（植骨组） 各治疗患者30例，以Frankel分级、VAS评分、椎体压缩率、Cobb角等为观察指标， 根据患者术前、术后2天和术后1年的结果，分析两种手术方法的临床疗效，并作系 统比较。 结果：60例患者术后疼痛均明显缓解，多数患者功能障碍恢复良好；术后椎体高 度和后凸畸形均较术前得到明显改善。不植骨组和与植骨组相比，术后2天椎体压缩 率及Cobb角比较均无统计学差异 (P0.05),术后1年椎体压缩率和Cobb角比较均有统 计学差异(P0.05)。 结论：1、两组手术方法均可有效缓解患者的疼痛并可较好的恢复其功能障碍， 椎体高度和形态均得到明显改善；2、植骨组可防止后期椎体高度和矫正角度的丢失， 临床疗效较不植骨组更为满意。 关键词 胸腰段爆裂骨折；内固定；植骨 The Clinical Results Analyse of Pedicle Fixation alone and Pedicle Fixation Combined with Bone Graft Via Pedicle for the Treatment of Thoracolumbar Burst Fractures Speciality: Orthopedics of TCM Author: Liu Tong Tutor: Prof. Hao Yanke ABSTRACT Objective: Analysis comparison of pedicle screw internal fixation system alone and pedicle screw internal fixation system combined with bone graft of vertebral body via pedicle for the clinical treatment of thoracolumbar (T11 ~ L2) burst fractures.
