腋窝皱襞切口薄皮瓣法治疗腋臭探究【摘要】目的探讨腋窝皱襞切口直视剪除大汗腺至薄皮瓣法治疗腋臭的疗效。方法收集近年来腋臭患者197例,采用腋窝皱襞切口394个,手术范围超腋毛分布1 cm,肿胀麻醉,直视下剪除包含大汗腺的腋部皮下组织5 mm左右,直至手术范围皮肤形成真皮下血管网皮瓣。创面用双极电凝彻底止血,术后引流,加压包扎等措施,7 d拆线切口愈合。结果臭味标准参考Tung-Chain腋臭疗效评价标准评定。治愈387个腋窝,占97.97%;显效7个腋窝,占2.03%;无效:0个。腋毛变化:明显减少368个,占90.34%;一般减少,19个,占9.66%;减少不明显,0个。出汗状况:明显减少342个,占86.80%;改善,但不明显52个,占13.20%;无变化0个。手术恢复时间平均7 d左右。并发症:皮肤边缘坏死8个;切口瘢痕3个;血肿1个;肩关节活动受限0个。结论腋窝皱襞切口直视下清除大汗腺至腋部皮肤薄皮瓣法治疗腋臭,具有根除腋臭病因的解剖与病理学基础,具有操作简单,疗效可靠,创伤小,并发症少等特点,最大限度的符合整形外科原则,值得临床应用。? 【关键词】 腋臭;皮瓣;治疗 ??
