提 要 目的：通过运用视诊法、触诊法、电测定法三种常用耳穴诊断方法对腰椎间盘突 出症患者及健康人的耳穴进行观察并作出诊断，得出三种耳穴诊断方法诊断腰椎间 盘突出症是否均有效，三者之间是否存在差异；利用所探测的耳穴阳性反应点进 行耳穴割治治疗腰椎间盘突出症，通过统计学分析，得出耳穴割治法治疗腰椎间盘突 出症是否有效，并进一步印证耳医学是否可以应用于腰突症的诊断及治疗。 方法：临床随机选择腰椎间盘突出症患者 60 例，无腰椎间盘突出症症状及体征 的健康人 30 例。运用视诊法、触诊法、电测定法三种常用耳穴诊断方法，对腰椎间 盘突出症患者及健康人的耳穴进行观察，并对所得数据进行统计学分析，得出结论。 运用耳穴割治法治疗腰椎间盘突出症患者，依据日本骨科学会腰背痛手术评分标准 [1] (JOA) ，对症状改善情况进行评分比较，经统计学分析，得出结论。 结果： 1．视诊法、触诊法、电测定法探测耳穴阳性反应点，观察组与对照组得分经 T 检验，均为 P 0.01，具有极显著性差异。 2．观察组组内三种不同耳穴诊法之间通过秩和检验两两比较，均为 P0.017， 具有显著性差异。 3．耳穴割治治疗组与常规针刺对照组治疗后疗效评价经 T 检验，P0.01，具有 极显著性差异。 结论： 1.视诊法、触诊法、电测定法对诊断腰突症均具有特异性；2.耳穴电测定法阳性 穴位病理程度最强，触诊法强于视诊法；3.耳穴割治法结合常规针刺治疗腰突症疗效 优于常规针刺。 关键词 耳穴诊治；腰椎间盘突出症；临床观察 Clinical observation of auricular diagnosis and treatment for sixty cases with Lumbar Disc Herniation Speciality: Acupuncture and Tuina Author: Zhang Lili Tutor: Prof. Ma Qijiang Abstract Objective: With comparative observation between patients with Lumbar Disc Herniation and healthy people diagnosed by three common ear diagnosis methods, the visual examination method,palpation and electrical determination method,to obtain the clinical effects whether the three methods have the difference or not;To explore the therapeutic effect of treating the lumbar disc herniation by “cuting therapy” at the auricular reaction points .
