钙拮抗剂在心血管疾病中不良反应研究【摘要】 目的 探讨钙拮抗剂在心血管疾病中的主要不良反应与防范对策。方法 回顾性分析钙拮抗剂的主要临床应用情况与出现的不良反应。结果 240例使用钙拮抗剂治疗的心血管疾病患者中，发生不良反应的有38例，发生率为15.8%。多为硝苯地平和尼莫地平引起，最常见的不良反应是脸红、头痛、眩晕。结论 钙拮抗剂在心血管疾病的治疗中，占有非常重要的地位。但是应根据钙拮抗剂的药理作用、剂量、剂型、患者的个体差异及年龄选择适宜的药物，避免不良反应。 【关键词】 钙拮抗剂；心血管疾病；不良反应 The Analysis of Adverse Reactions of Calcium Antagonists in Cardiovascular Disease LIU Li-hua. Results There were 38 cases of adverse reactions in the 240 patients treated with calcium antagonists in cardiovascular disease, the rate was 15.8%.Mostly caused by nifedipine and nimodipine, the most common adverse events were flushing, headache, dizziness.Conclusion Calcium antagonists in the treatment of cardiovascular disease plays a very important role.But it should be based on the pharmacological effects of calcium antagonists, dose, dosage form, the patient?s individual differences and age for appropriate choice of drugs to avoid adverse reactions. 【Key words】 calcium antagonist; cardiovascular disease; adverse reactions 当前钙拮抗剂广泛应用于心血管疾病的治疗，如心律失常、高血压、心肌缺血性和慢性心功能不全等疾病。近年来，应用范围有很大发展［1］。同时，由于钙拮抗剂对人体的不良反应也多种多样，临床应用存在争议［2］。因此，应采取有效措施预防和减少不良反应的发生，使钙拮抗剂在心血管疾病中发挥最佳治疗作用。 1 资料与方法 调查2008年到2010年我院240例使用钙拮抗剂治疗的心血管疾病患者，其中男160例，女80例；年龄11~79岁，其中50岁以上200例。使用药物有6种：维拉帕米、硝苯地平、地尔硫卓、尼莫地平、非络地平、氟桂嗪。统计不良反应发生情况。 2 结果 240例使用钙拮抗剂治疗的心血管疾病患者中，发生不良反应的有38例，发生率为15.8%。在排除其他原因引起的情况，全部为服用维拉帕米、硝苯地平、地尔硫卓、尼莫地平4种钙拮抗剂后引起，服用非络地平与氟桂嗪的患者没有发生不良反应。其中硝苯地平引起不良反应20例，尼莫地平引起不良反应10例，维拉帕米引起不良反应5例，地尔硫卓引起不良反应3例。不同药物造成的不良反应表现情况见下表。 3 讨论 3.1 钙拮抗剂的常用药物 1987年世界卫生组织将钙拮抗剂的常用药物分为选择性和非选择性两大类，在选择性钙离子阻滞剂中又分为：维拉帕米类、硝苯地平类和地尔硫卓类三种。而非选择性的钙离子阻滞剂不用于抗高血压治疗［3］。1992年国际药理学联合会认为常用于抗高血压治疗的钙离子阻滞剂，选择性地作用于L-型钙离子通道，结合部位在α1
