78例瘢痕子宫妊娠阴道分娩临床研究[摘要] 目的 通过对瘢痕子宫妊娠分娩方式的临床观察，探讨其可行性。 方法 回顾性分析2011年1月～2012年1月所收治的183例瘢痕子宫妊娠者，选择符合阴道试产条件的78例为观察组，选取同期有阴道试产条件并要求阴道试产的80例初产妇为对照组，观察两组第一产程时间、第二产程时间、产后出血量、新生儿窒息情况。结果 观察组78例中，成功阴道分娩61例，约占78%，母子健康。其中第一产程时间为（9.51±0.64） h、第二产程时间为（0.98±0.43） h、产后出血量为（156±23） mL、新生儿窒息1例。与对照组80例相比，差异无统计学意义，P 0.05。 结论 瘢痕子宫妊娠阴道分娩具有一定可行性，可降低剖宫产率。 [关键词] 瘢痕子宫；妊娠；阴道分娩；分娩方式 [中图分类号] R714.3 [文献标识码] B [文章编号] 1674-4721（2012）07（a）-0249-02 Clinical analysis of vaginal delivery of pregnancies in uterine scar in 78 cases ZHANG Zhonghua Department of Gynaecology and Obstetrics, Family Planning Service Station of Sui County in Shangqiu City, He’nan Province, Suixian 476900, China [Abstract] Objective To explore the feasibility through clinical observation of the delivery mode of pregnancies in uterine scar. Methods One hundred and eighty three cases of scar uterine pregnancies women treated in our hospital from January 2011 to January 2012 were analyzed retrospectively. 78 cases accorded with vaginal trial-produce conditions were selected as the observation group and at the same period 80 cases who had vaginal trial-produce conditions and required vaginal trial-produce were selected as the control group. Time of the first stage of labor, the second stage of labor, the amount of postpartum bleeding, neonatal suffocation of the two groups were observed. Results Among 78 cases of the observation group, 61 cases of vaginal delivery were successful, accounted for 78%, the mothers and children were all healthy. The time of the first stage of labor was (9.51±0.64) hours, the time of the second stage of labor was (0.98±0.43) hours, the amount of postpartum hemorrhage was (156±23) mL, and there was 1 case suffered with neonatal asphyxia; compared with the control group, there were no significant differences (P 0.05). Conclusion Vaginal delivery of pregnancies in uterine scar has certain feasibility, which can reduce the cesarean section rate. [Key words] Uterine scar; Pregnancy; Vaginal del
