克氏针内固定及石膏外固定治疗儿童肱骨髁上骨折疗效研究[摘要] 目的 探讨儿童肱骨髁上骨折手术克氏针的应用。方法 回顾性分析我院2005年1月～2006年10月收治的326例儿童肱骨髁上骨折，将其随机分为克氏针组216例及手法复位石膏外固定组110例，对两种方法的临床疗效、影响因素、并发症等进行分析。结果 总体疗效克氏针组优161例，良36例，优良率91.2%；手法复位组优78例，良19例，优良率88.2%。克氏针组与手法复位组患儿的总体疗效无显著性差异（P＞0.05）,骨折的类型与患者的恢复程度也不存在显著性差异，发生肘内翻分别为21例和19例，克氏针组效果好于手法复位组（P＜0.01）。结论 采用闭合复位克氏针内固定法治疗小儿肱骨髁上骨折，创口小，易操作，骨折的固定效果好，值得临床推广。 [关键词] 肱骨髁上骨折；克氏针；手法复位；肘内翻 [中图分类号] R726.8[文献标识码] B[文章编号] 1673-9701（2011）21-54-03 Efficacy of Kirschner Wire Fixation and Cast Immobilization in Treatment of Children with Supracondylar Fracture of Humerus LIN Rui Department of Orthopedics, the People’s Hospital of Wencheng County in Zhejiang Province, Wencheng 325300, China [Abstract] Objective To explore clinical therapeutic effect of supracondylar humerus fractures by reduction internal fixation with Kirschner wire in children. Methods We retrospectively analyzed 326 children who had humeral supracondylar fractures treated in our department from January 2005 to October 2006. The 326 cases were randomly divided into two groups of 110 cases in the control group and 216 cases in experimental group. The patients of experimental group were treated by reduction internal fixation with Kirschner wire and the others were treated by manipulative reduction with closed-plaster. The therapeutic effect, influencing factors and complications were compared. Results The group of closed reduction and cross percutaneous pinning had excellent results in 161 cases, good outcomes in 36 cases, excellent-good rate was 91.2%. The group of closed reduction had excellent results in 78 cases, good outcomes in 19 cases, excellent-good rate was 88.2%. There were no significant differences of statistical significance（P＞0.05）. Types of fractures and therapeutic effect were no significant differences of statistical significance.
