脐血血气评价新生儿窒息临床探究[摘要] 目的 探讨新生儿脐血血气分析与新生儿围生期窒息关系。方法 选择60名正常新生儿作为对照组，选择120名窒息新生儿作为窒息组，窒息组新生儿按窒息严重程度分为轻度窒息组及重度窒息组，采集脐动脉血进行血气分析，比较各组新生儿血气分析指标差异。结果 窒息组新生儿血气分析pH、PO2及BE低于对照组，PCO2高于对照组，重度窒息组血气分析pH、PO2及BE低于轻度窒息组，PCO2高于轻度窒息组，差异具有统计学意义。结论 血气分析指标能够较为准确的评价新生儿窒息及其严重程度，为新生儿窒息的诊断及及时抢救提供参考依据。 [关键词] 新生儿窒息；脐动脉血；血气分析 [中图分类号] R722.12[文献标识码]B[文章编号] 1673-9701（2011）25-65-02 Clinical Study of Blood Gas Analysis in Cord Blood on Neonatal Asphyxia TANG Yuxin SUN Yunming Department of Gynaecology and Obstetrics，Maternal and Child Health Hospital of Zhoushan City in Zhejiang Province，Zhoushan 316000，China [Abstract] ObjectiveTo investigate the correlation between blood gas analysis in cord blood with neonatal asphyxia and pathogenetic condition. MethodsAll 60 cases normal newborn were selected into control groups and 120 cases newborn with neonatal asphyxia were selected into asphyxia groups，and asphyxia groups were divided into longitudinal aspHyxia groups and severe asphyxia groups， blood gas analysis in cord blood were detected in all patients， indicatrix difference of blood gas analysis were contrasted. Results The level of pH，PO2 and BE were lower and level of PCO2 was higher in asphyxia groups than the control groups，level of pH，PO2 and BE were lower and level of PCO2 was higher in severe asphyxia groups than in longitudinal aspHyxia groups. Conclusions Blood gas analysis of cord blood could exactly evaluate neonatal asphyxia and pathogenetic condition，and provide reference for diagnosis and therapia ofneonatal asphyxia. [Key words] Neonatal asphyxia； Cord blood； Blood gas analysis 窒息是新生儿围生期严重的并发症之一，窒息能够引起新生儿体内血氧饱和度急剧下降，氧供不足，新生儿脑功能损伤及智力发育异常，严重者甚至导致新生儿临床死亡，早期诊断对其乏氧程度的判断及纠正对于新生儿窒息的治疗具有重要的意义[1]，本文就脐动脉血血气分析对新生儿窒息的诊断价值及其对病情的评估作用进行研究，现报道如下。 1 资料与方法 1.1 临床资料 选择我院2008年1月～2010年12月产科分娩的正常新生儿60例作为对照组及新生儿窒息患儿120例作为窒息组，两组患儿胎龄37~42周，出生体重2600～4200g，阴道分娩84例，剖宫产96例。正常新生儿1min及5min Apgar评分8～10分，且无宫内窒息史，窒息组新生儿依据Apgar评分进行诊断，Apgar评分≤7分，并按Apgar评分进行分级，其中Apgar评分4～7分为轻度窒息，≤3分为重度窒息，各
