Method: Analyzing the cases of CSM during 4 years ( from October, 2007 to January, 2011 ) treated by two posterior operative ways in Spine Department, Affiliated Hospital of Shandong University of Traditional Chinese Medicine, the author collected 40 patients with complete date and meeting the criteria after the surgery by means of telephone, mail and out-patient and then divided them into group A and group B. Group A includes 19 cases underwent laminectomy and lateral mass screw fixation, and group B includes 21 cases of received open-door laminoplasty. The follow-up period was 6-72 months, in which the author compared the improvement rate of the both groups and evaluated the two curative effect according to pre- and postoperative spinal cord JOA scores. Meanwhile, postoperative cervical curvature index and axial symptoms were also analysed in the article. Result: Comparison of JOA scores, postoperative cervical curvature index and axial symptoms shows that the change of pre- and postoperative spinal cord JOA scores has no difference between the group A and B. However, cases underwent laminectomy and lateral mass screw fixation has a good clinical outcome and a preventive effect on the development of postoperative axial symptoms.
