静脉环磷酰胺冲击治疗狼疮肾炎的长期随访（Long term follow-up of intravenous cyclophosphamide pulse therapy for lupus nephritis） Long term follow-up of intravenous cyclophosphamide pulse therapy for lupus nephritis 2011-06-14 Dai min Chen Shunle Bao Chunde Abstract: Objective To investigate the long-term efficacy and toxicity of intravenous cyclophosphamide in lupus nephritis. Methods 66 cases of lupus nephritis were treated with intravenous cyclophosphamide (CTX 0.5 to 1 g/m2, 1 times a month for 6~12 months). The urine protein, renal function and sex hormone levels were observed. Results urine protein decreased from (4.54 + 2.98) g/d to (1.49 + 1.26) g/d, effective in 59 cases (89.4%). After a follow-up of 1~9 years (mean 4.3 years), 63 patients (96.9%) maintained serum creatinine levels of less than 140 mol/L. 7 cases were postmenopausal for more than 6 months, accounting for 14.9% of 47 women of child-bearing age. In 13 women, the levels of LH, FSH, E2 and P were measured. In 2 premenopausal women, progesterone (P) did not show a peak change in luteal phase, 1 of which were postmenopausal after 2 months. Conclusion intravenous cyclophosphamide pulse treatment of lupus nephritis has a significant curative effect, up to 86.4%, and less toxic side effects, patients are easy to tolerate. The monitoring of progesterone in the corpus luteum at regular intervals is likely to detect the toxicity of CTX to the ovary in time and adjust the medication to prevent ovarian failure.
