正畸减数前磨牙结合II类牵引治疗对颅颌面结构影响[摘要]目的：评价减数14、24、35、45，并使用II类牵引关闭拔牙间隙的矫治方法对上下颌骨的影响。方法：选择广东省口腔医院正畸科患者62例（男21例，女41例），年龄（17.5±4.6）岁。所有患者均采用拔除14、24、35、45，结合双侧轻力（50～100g）II类牵引的治疗模式。对治疗前后的X线头影测量测量值进行比较。结果：治疗后下颌平面角和咬合平面倾斜度均有增加（P＜0.05）；前后面高虽然有所增加（P＜0.05），但二者比率无统计学改变；下颌有效长度增加、下颌上角减小和下颌下角增大（P＜0.05）。结论：拔除14、24、35、45，结合轻力II类牵引调整上下颌骨及牙齿咬合关系的治疗方案，可造成下颌后下旋转，但尚未破坏前后面部的垂直向比率。 [关键词]头影测量分析；拔牙；颌间牵引 [中图分类号]R783.5 [文献标识码]A [文章编号]1008-6455（2010）02-0282-03 Cephalometric assessment of jawbone changes in patients treated with extraction of 2 maxillary frist premolars and 2 mandibular second premolars combined with class II intermaxillary elastics LI Shu-jun1,LAN Ze-dong2 （1.Department of Orthodontics，Guangdong Provincial Stomatological Hospital and the Affiliated Stomatological Hospital of Southern Medical University,Guangzhou 510280,Guangdong,China;2.Dental Hospital of Guangzhou Medical College） Abstract:ObjectiveThe purpose of this study was to evaluate the jawbone changes in patients treated with extraction of 2 maxillary frist premolars and 2 mandibular second premolars combined with classⅡ intermaxillary elastics.MethodsThe sample included 62 patients (male 21,female 41),the mean age is 17.5±4.6 years.All the patients were treated with extraction of 2 maxillary frist premolars and 2 mandibular second premolars combined with class II intermaxillary elastics. Lateral cephalometric radiographs were taken at pretreatment and posttreatment,T tests were used to compare the sagittal and vertical changes between maxillary and mandibular. ResultsAnalysis of the date showed statistically significant increases of anterior,posterior face heights and the mandibular plane angle(P＜0.05) ,but no significant changes were observed for rate of the face height(P＞0.05).ConclusionThe treatment may bring a light downward rotation of the mandibular plane ,but it does not cause an unfavourable effect on rate of the face height.
