· 1 12 · 中华护理杂志2012年2月第47卷第2期Chin J Nurs,February 2012,Vol 47,No.2 骨科椎管内麻醉患者术后首次饮食时间的研究 郭延霞 高洁 史建秀【摘要】 目的探讨骨科椎管内麻醉患者术后首次饮食的适宜时间。方法在山东省淄博地区3家医院骨科选择520例骨科椎管内麻醉患者作为研究对象,以病区为单位分为观察组和对照组,每组260例。观察组术后有饮食需求时,如果意识清楚、无胃部不适即可先饮水少量,再进食;对照组则按常规术毕6h后才可进饮食。分别于术后5h和进食后2h评估两组患者口渴、饥饿感、恶心、呕吐、腹胀等情况,并进行比较。结果术后5h对照组自觉有口渴、饥饿感的人数高于观察组(P<O.01)。两组进食后2h各项观察指标差异无统计学意义(P>O.05)。结论骨科椎管内麻醉患者术后6h进饮食不应是惟一标准,评估患者的主观感受和客观体征后,根据患者需求术后提早开始饮食是可行的。【关键词】 手术后医护;麻醉,脊椎; 饮水; 进食Study On the proper time of postoperative oral feeding in patients undergoing orthopedic surgery under spinal anes·thesia/GUO Yan-xia.GAO Jie.SHI Jian-xiu【Abstract】Objective To investigate the proper time of postoperative oral feeding in patients undergoing orthopedic surgeryunder spinal anesthesia.Methods Five hundred and twenty patients undergoing orthopedic surgery under spinal anesthesiawere recruited from three hospitals in Zibo area of Shandong Province.The patients were divided into the experimental groupand the control group.The patients in the experimental group were allowed to take food once they had needs to eat and re—covered to normal consciousness without stomach discomfort.The patients in the control group were not allowed to take fooduntil 6 hours after operation.The symptoms such as thirst,hunger,nausea,vomiting,abdominal distention at five hours after op—eration and two hours after eating were compared between the two groups.Results There were significant differences in thesymptoms of thirst and hunger between the two groups at five hours after operation(P<0.01).There was no significant differ-ence in any symptoms between the two groups at two hours after eating.Conclusions Early oral feeding after operation isfeasible according to the individualized demands.【Key words】Postoperative Care;Anesthesia,Spinal;Drinking;Eating 为配合手术、麻醉的顺利进行.减少并发症的发生,手术患 术后首次饮食时间进行了探讨.旨在为临床护理实践提供依者术前、术后往往需要禁饮食。按照常规,骨科椎管内麻醉患 据,为制汀和完善术后进饮食时间的标准提供参考。者.术前禁食12h,术后再禁食6hnj,手术当天16~21h为禁食时间,由此给患者带来一定的影响。美国的麻醉医师协会重新制 1对象与方法订了禁饮食的指导方案,我国也有护理教科书提出,非腹部手 1
