血液透析及持续不卧床腹膜透析患者脑卒中发生及分析[摘要] 目的:探讨间歇性血液透析(IHD)患者和持续不卧床腹膜透析(CAPD)患者的脑卒中发生情况以及相关因素的分析。方法:收集2007年1月～2009年12月于我院行慢性透析治疗的尿毒症患者共150例,其中IHD患者80例,CAPD患者70例,分析这些患者脑卒中的发生情况,比较脑卒中的发生率。结果:80例IHD患者中,13例发生脑卒中(16.3%),其中出血性脑卒中5例(6.3%),缺血性脑卒中8例(10.0%)。70例CAPD患者中,7例发生脑卒中(10.0%),出血性脑卒中1例(1.4%),缺血性脑卒中6例(8.6%)。结论:慢性透析患者脑卒中发生率高,而IHD患者脑卒中发生率又明显高于CAPD组患者。 [关键词] 血液透析;持续不卧床腹膜透析;脑卒中 [中图分类号] R692.5[文献标识码]A [文章编号]1673-7210(2010)06(a)-054-02 Effect of the hemodialysis and continuance ambulatory peritoneal dialysis on stroke patients AN Kun, ZHANG Xiaoming (Department of Nephrology, the First People’s Hospital of Dalian City, Dalian116100, China) [Abstract] Objective: To investigate the stroke occurrence of chronic dialysis patients, including intermittent hemodialysis(IHD) patients and continuance ambulatory peritoneal dialysis (CAPD) patients. Methods: 150 patients with chronic dailysis from 2007 to 2009, including 80 cases of IHD patients and 70 cases of CAPD patients. Their stroke occurrences were analyzed. Results: Of 80 IHD patients, 13 cases (16.3%) experienced stroke, including 5 cases (6.3%) hemorrhage stroke and 8 cases (10.0%) ischemia stroke. Of 70 CAPD patients, 7 cases (10.0%) experienced stroke, including 1 case (1.4%) hemorrhage stroke and 6 cases (8.6%) experienced ischemia stroke. Conclusion: The incidence of stroke is quite high in chronic dialysis patients. The incidence of stroke in IHD patients is higher than incidence of stroke in CAPD patients. [Key words] Hemodialysis; Continuance ambulatory peritoneal dialysis; Stroke 慢性透析患者的心血管并发症非常普遍,是导致慢性透析患者死亡的重要原因之一。慢性透析患者脑卒中的研究较少,发生脑卒中的危险因素包括高血压、高血脂、动脉粥样硬化等,而慢性透析患者高血压,高血脂情况非常普遍。本文中笔者主要研究慢性透析患者脑卒中的发生率,以及相关因素和不同透析方式对脑卒中发病率的影响。 1 资料与方法 1.1 一般资料 选取2007年1月～2009年12月在我院行慢性透析治疗的尿毒症患者150例,随机分为两组。IHD组80例中,男40例,女40例;平均年龄(60.0±12.6)岁。CAPD组70例中,男40例,女30例;平均年龄(59.6±13.0)岁;透析时间3～4年。两组患者在吸烟、基础病、胆固醇、三酰甘油等指标方面差异无统计学意义(P0.05),具可比性。 排除标准:具感染、急性应激状态,既往具有中枢神经系统疾病以及心肌梗死病史的患者。脑卒中的定义根据2007年中国脑血管病指南标准:具中枢神经系统表现及脑影像学(脑CT或M
