民航飞行员冠心病搭桥术或支架植入术后特许飞行3例 金清清（200336，民航上海医院，上海） 姚永杰（200433，上海，海军医学研究所） 通讯作者：姚永杰 【摘要】目的?探讨民航飞行员冠心病搭桥术或支架植入术后的特许飞行的鉴定和原则。方法?报告3例民航飞行员冠心病搭桥术或支架植入术后的特许飞行病例，对飞行员的病史、冠心病危险因素、飞行情况和复查结果等进行分析，讨论民航飞行员冠心病搭桥术或支架植入术后的特许飞行的原则。结果?病例特点如下：1.总飞行时间长，工作经验丰富；2.年龄50岁以上；3.?冠状动脉80%以上狭窄，适宜进行冠状动脉搭桥术或支架植入术；4.手术成功，术后恢复良好，活动无影响；5.术后遵医嘱服用药物治疗，积极进行冠心病二级预防；6.术后血管桥畅通，未发生新生血管阻塞，检查均未见明显异常，放飞后飞行正常。结论 若飞行员冠心病搭桥术或支架植入术后：（1）一般状况和飞行耐力良好，无后遗症；（2）心血管疾病危险因素控制理想，无明显缺血症状及缺血客观证据；（3）可严格控制各项心血管危险因素、专科随访、定期复查、航医严密观察的前提下，可考虑批准特许飞行。 【关键词】心血管疾病；冠状动脉搭桥术；支架植入术；特许飞行 Three Cases of Waiver After Coronary Artery Bypass or Stent Implantation Jin Qingqing1, Yao Yongjie2 1 Shanghai hospital of civil aviation; 2 Naval Medical Research Institute of PLA * Corresponding author: Yao Yongjie (648319648@) . 【Abstract】Objective?To explore the evaluation and principles of waiver of commercial pilot after?coronary artery bypass surgery or coronary artery stent implantation.?Methods?Three cases of commercial pilot waiver after?coronary artery bypass surgery or coronary artery stent implantation are reported. The medical history, risk factors of coronary heart disease, flight condition and the physical reexamination results are anylazed.?Results?The characteristics of the cases are as follows: 1. The total flight time of these pilots is long and they have rich work experience; 2. Their ages are more than 50 years; 3. The coronary artery stenosis is more than 80%, which is suitable for coronary artery bypass surgery or stenting; 4. They had successful surgeries and good postoperative recovery, without imparied activity; 5. The had postoperative prescribed medication and grade two prevention of coronary heart disease; 6. The had no vascular occlusion or obvious abnormalities after surgery. The flights after surgery were sucessful. Conclusion?A waiver considering can be given after coronary artery bypass graft or stent implantation without sequelae if: (1) the vectim is in good general condition and has good flight endurance; (2) the vectim
