中国基层医药2010年7月第 17卷第 14期 ChinJPrimMedP ， ! ! !：!! ： · 1913 · 甘精胰岛素联合二 甲双胍或／和阿卡波糖治疗 新诊断2型糖尿病疗效观察 王 云 杜宁迁 邵 蔚 刘潇潇 武银铃 【摘要】 目的 比较甘精胰岛素联合二甲双胍或／和阿卡波糖治疗新诊断2型糖尿病的临床疗效及安 全性。方法 84例新诊断2型糖尿病患者先服用二甲双胍至少4周 ，血糖控制不佳的患者随机分为三组。A 组予二甲双胍加甘精胰岛素，B组予阿卡波糖加甘精胰岛素，c组予二甲双胍与阿卡波糖加甘精胰岛素治疗， 随访 12周。结果 三组治疗后空腹血糖 (FBG)、餐后2h血糖 (2hBG)、糖化血红蛋白(HbA1c)均低于治疗前 治疗，A、B两组HbA1c部分达标，c组 HbA1c全部达标 (P0．05)。C组疗效优于A、B组 ，达标时间最短，日 胰岛素用量最少。A、B组BMI无增加，C组 BMI有所下降(P0．05)。三组低血糖发生率均l1％，无严重 低血糖事件发生，差异无统计学意义(P0．05)。结论 甘精胰岛素联合二甲双胍或／和阿卡波糖均能较好 地控制血糖 ，对体质量影响小，低血糖发生率低，三药联合疗效优于二药联合 ，总体安全、有效、方便 ，是新诊断 2型糖尿病理想的治疗方案。 【关键词】 甘精胰岛素；二甲双胍；阿卡波糖；2型糖尿病 TheeffectofGlargineinsulincombined with Metforminor／and Acarboseintreatmentforthenewlydiag— nosedtype2diabetes WANGYun。DUNing—qian．SHAOWei。LIUXiao—xiao．UYin—ling．DepartmentojEndocri： nology，TheGeneralHospitalofHuainanEasternHospitalGroup，Huainan，Anhui232001，China A【bstract】 objective Tocomparetheclinicaleffectsandsafetyoftreatingnewlydiagnosedtype2diabetes patientsbycombiningGlargineinsulinwithMetforminor／andAcarbose．M ethods 84patientswithnewlydiagnosed type2diabetes，whohaveahistoryofweightloss，bodymassindex(BMI)23～28kg／m andpoorlyeontroUedblood ucoseinMefforminatleastamonth，wererandomlydividedintothreegroups．GlargineinsulinplusMefforminwere usedingroupA．GlragineinsulinplusAcrabose(withoutmetformin)wereusedinrgoupB，Glargineinsulin，Acarbos nadmetforminwereusedin Croup C．M 1threegroupsweregivena12一weekfollow—up．Results FBG，2hBG and HbAl。levelswerelowerinthepatientsaftertreatment(P0．05)．HbAi。ofpatientswaspa~lyuptothestandardin groupA andrgoupB，butHbAlofpatientswasalluptothestandardingroupC．TheeffectsingroupCwerebetter thanthoseingroupA andgroupB．Ithadtheshortesttimetoachievethetargetnadthesmallestdoseofinsulinin groupC．BMIinrgoupAandBdidnotchangebutdecreasedingroupC(P0．05)．Theincidenceoflowbloodsugar inallthreergoupswaslowandnosignificantdifferenceobservedamongthreergoups(P0．05)．Andinallthree rgoupsnoseverehypoglycemiao
