摘要 基本医疗保障是社会保障体系的重要组成部分，也是医改的重要领域之一。 基本医疗保险制度是筹集、开发、配置和利用卫生资源的一种制度安排。对于保 障劳动者的身体健康，抵御疾病风险有着极其重要的作用。 医疗保险费用支付方式是指社会医疗保险机构对为参保人提供医疗服务的 医疗机构进行经济补偿的方式、方法，利用卫生经济学理论来促进资源的公平利 用。它作为一种特殊的激励手段，对医疗费用的控制起着关键作用，影响着整个 医疗服务体系的效果。 医疗保险费用支付方式包括按服务项目付费、总额付费、按服务单元付费、 按人头付费、按病种付费等。目前，世界各国几乎都把改革医保支付方式作为改 革医疗保险制度的突破口和控制医疗费用不合理增长的主要手段。医疗保险费用 支付方式改革趋势已经由后付制向预付制、由单一型向混合型、由自由分散向统 一集中、由限制性控制向诱导性控制的方向发展。 我国基本医疗保险制度在定点医疗机构采取不同的支付方式，总体上具有 客观性、未知性、波动性和风险性等特点，每种方式都有其优势和缺陷。衡阳市 医疗保险费用支付方式共经历了三次变革。2001 年开始实行按项目付费；2003 年起，采用以平均住院人次付费为主，以按单病种费用支付和按项目付费为补充 的复合型方式结算；2009 年 4 月开始，形成了以总额预算式为主，按单病种付 费和按项目付费相结合的混合式支付体系。每个阶段的变革对费用控制和医疗机 构运行都产生过积极的影响。在医改试点以及发展过程中，衡阳一直将医疗保险 费用支付问题作为一个关键不断探索和改革，但仍然存在自付费用计算不全、考 核标准欠科学、监管力度不到位等问题。 完善医疗保险费用支付方式必须遵循“以收定支、量入为出”的原则。通过 改善医保支付制度与支付方式、建立科学合理的病种付费标准、实行城乡统筹和 费用即时结算、加强对医保基金运行的监管力度等途径，有效控制医疗费用的不 合理增长，使医疗保险制度的功效得到最大程度的发挥。 1 Abstract Basic Medical Security is an important component of Social Security System, it is also one of the most important areas of medical reform.
