精品论文 参考文献 7358例患者输血前、术前HBV、HCV、TPPA、HIV抗体检测结果的调查分析 王浩斐 （辽宁省抚顺市中心医院检验科 113006） 【摘要】目的 调查分析住院及门诊患者，手术前、受血前及做胃镜、肠镜前，乙肝、丙肝、梅毒HIV抗体的筛查和检测，减少和避免医院内感染和医疗纠纷的发生。方法 采集7358例门诊及住院患者，术前及住院患者术前及受血前的静脉血，用酶联免疫法检测乙肝两对半，抗-HCV和血清中HIV抗体，用郑州安图全自动酶标仪检测其OD值，用胶乳凝集试验做TPPA，阳性者用甲苯胺红不加热血清试验（TRUST），倍比稀释测血清滴度。结果 7358例患者术前、受血前、胃镜、肠镜检查前的血样中，其中HIV抗体阳性的4例，均为省疾控中心确认，梅毒抗体阳性的14例，男9例，女5例，其中有3例是1-20天的新生婴儿，母婴垂直传播。HBSAg阳性的508例，抗-HCV阳性的67例。血透科HBSAg感染率14.75%，HCV感染率3.48%；消化科HBSAg感染率10.27%，HCV感染率3.64%；外科HBSAg感染率8.11%，HCV感染率0.57%；其中HIV感染的患者1例有出国史，并且通过性传播传给妻子，1例去过河南HIV高感染区，一例为消化科贫血患者，经输血感染，TPPA有3例新生儿，通过母婴垂直传播，HCV感染的有有输血史、拔牙史的。结论 本调查对7358例患者术前、受血前、肠镜、胃镜检查前采集静脉血进行HBV、HCV、TPPA、HIV抗体的检测，把检测结果存入病案，作为患者的原始记录，为临床提供及时的诊断依据，同时为医患双方减少了不必要的纠纷，为医疗纠纷提供法律依据，对切断传播途径，防止交叉感染，是十分必要的。 【关键词】 输血 手术 HBV HCV TPPA HIV 抗体 【中图分类号】R446.11 【文献标识码】A 【文章编号】2095-1752（2013）13-0120-02 Fushun city Liaoning prorince center hospital clinical laboratory In 7358 patients before transfusion before operation, HBV HCV TPPA HIV antibody test results of analysis 【Abstract】Objective To investigate the hospital and outpatients prior to surgery, done by the blood before and gastroscopy, colonoscopy before, hepatitis B, hepatitis C, syphilis, HIV antibody screening and testing, to reduce and prevent hospital infection and the occurrence of medical disputes .
