提 要 目的：比较夹板外固定、外固定架固定和切开复位T型钛板内固定三种不同治疗 方法在Barton骨折治疗中的临床疗效及选择应用。 方法：从山东中医药大学附属医院骨科选择 2010 年 6 月～2011 年 12 月期间采 用夹板外固定、外固定架固定和切开复位 T 型钛板内固定治疗的 Barton 骨折病例共 35 例，从骨折愈合时间、腕关节解剖评估和腕关节功能等方面对三种治疗方法进行 比较和分析。 结果：A、B、C三组术后1个月腕关节解剖评估优良率分别为41.67%、81.82%、 83.33%，术后 3 个月腕关节功能评估优良率分别为 50.00%、90.91%、91.67%。经统 计学分析，B、C两组优于A组（P0.05），B、C两组之间无显著性差异（P0.05）。 结论：外固定架固定和切开复位T型钛板内固定治疗Barton骨折均取得了良好效 果；切开复位T型钛板内固定使骨折达到解剖复位并维持稳定固定，降低了腕关节后 期并发症的发生，可早期功能锻炼，是临床上治疗Barton骨折较为理想的方法。 关键词 Barton骨折，固定方法，疗效评价 Clinical analysis of three different fixation methods for the treatment of Barton fracture Speciality: Orthopedics of TCM Author: Liu ruizhong Tutor: Li jingyin Abstract Objective: Comparing splint external fixation, external fixation and opening reduction and internal fixation with T type titanium plate of three different treatment methods in Barton fracture treatment in clinical efficacy and selection of application. Method: From the orthopaedics in Affiliated Hospital of Shandong University of Traditional Chinese Medicine during 2010 March~2011 December, using splint external fixation, external fixation and internal fixation with T type titanium plate fixed in Barton fractures in 35 cases, From the time of fracture healing, the anatomy of the wrist joint assessment and wrist joint function compared and analyzed the three fixation methods. Result: The excellent and good rate of wrist joint anatomical assessment in group A,B and C was 41.67%,81.82%,91.67% respectively after one month. The excellent and good rates of wrist function assessment in group A 、B and C was 50.00% 、90.91%、91.67% respectively after three months.
