Operating and Support Hazard Analysis (OSHA) Conduct during design development phase – SEMP 2 and SEMP 3 Using HAZOP / SWIFT technique Procedures generally follows PHA with focus on the operating and supporting task including isolation and maintenance of equipment Recommend actions to eliminate or control associated risk to an acceptable level Design Team Input: Participate in hazard identification workshop to identify credible hazards Provide relevant design information / drawings / supporting task list / OM procedures (where available), and act as subject matter experts Consider and integrate proposed mitigation measure into design and advise status of implementation * Safety Integrity Level (SIL) Analysis Conduct during design development phase – SEMP 2 and SEMP 3 Assign SIL to all safety related system software Based on various requirements and above hazard analyses Allow decision on appropriate design and development techniques to be made Potential benchmarking with SIL of other rail system To be covered under software analysis Design Team Input: Provide relevant design information/ drawings and act as subject matter experts Designer to provide evidence to confirm that the software system design development process complies with relevant requirements. * Common Hazards Related to Rolling Stock Electrical Overheating Tensioned surfaces/ points Ignition of combustibles Induced voltage Electrostatic discharge Inadvertent activation Electrical arc Mechanical Sharp edges / points Lifting weights Vibration Rotating equipment Stability / toppling potential Altered structural properties Reciprocating equipment Ejected / thrown parts / fragments Pinch points Crushing surface * Common Hazards Related to Rolling Stock Pneumatic / hydraulic pressure Over pressurization
