内窥镜与常规切开法采集大隐静脉组织学比较 　　[摘要] 目的：对比内窥镜和常规切开两种方法采集的大隐静脉组织学变化。方法：选取2010年1月~2011年1月于我院行冠状动脉旁路移植术患者110例，分为内窥镜静脉采集组（EVH组，48例）和常规切开静脉采集组（CVH组，62例），比较两种静脉采集方法的切口局部并发症情况，光镜下观察静脉管壁的结构完整性，应用扫描电镜观察静脉内膜，评估内膜损伤情况。结果：两组患者切口并发症的发生率差异有统计学意义（P＜0.05），EVH组患者切口并发症发生率明显低于CVH组（P＜0.05），光镜和扫描电镜观察两种方法采集的血管组织学差异无统计学意义（P0.05）。结论：EVH组对静脉壁的损伤情况与CVH组相似，但能够明显减少患者伤口并发症的发生率。 　　[关键词] 冠状动脉旁路移植术；大隐静脉；内镜 　　[中图分类号] R654.4 [文献标识码] Ａ [文章编号] 1673-7210（2011）12（a）-064-03 　　 　　Comparison of histological alteration between endoscopic saphenous vein harvesting and conventional vein saphenous harvesting 　　HUANG Jinhong, DONG Jiande, GAO Feng, LI Zhijian, TENG Hale, ZHI Yonghui, CUI Shuisen, WAN Feng 　　Department of Cardiothoracic and Vascular Surgery, Beijing Electric Power Teaching Hospital of Capital Medical University, Beijing 100073, China 　　[Abstract] Objective: To compare the histological alteration between endoscopic saphenous vein harvesting and conventional saphenous vein harvesting. Methods: 110 cases of patients who were underwent coronary artery bypass grafting from January 2010 to January 2011 in our hospital were selected and divided into endoscope vein harvesting group (EVH group, n=48) and conventional vein harvesting group (CVH group, n=62). Incision local complications of the two collecting methods were compared. Light microscopy were used to assess the integrity of the vascular wall, and scanning electron microscopy were used to assess the trauma of the epithelium. Results: The incidence of incision complications were compared between EVH group and CVH group, the difference was statistically significant (P＜0.05), and the incidence of EVH group was lower than that of CVH group(P＜0.05). Vascular histological alteration was compared between the two groups by the observation of light microscopy and scanning electron microscopy, and the difference was not statistically significant (P0.05).
