CT与MRI对6例成人型环状胰腺诊断的对比分析 李 懋1，何晓静1，赵建农1，谢微波1，李炫飞2 （400010 重庆，重庆医科大学附属第二医院：放射科1，肝胆外科2） [摘要] 目的 探讨成人型环状胰腺的CT及MRI表现，以提高对该病的认识。方法 回顾性分析我院6例成人型环状胰腺病例，其中3例行CT腹部平扫及增强扫描；另外3例行MR腹部平扫及增强扫描，1例结合MRCP检查。详细分析每例影像学图像及其表现，最终诊断由2位高年资医师盲法观察，得出共同结果。结果 CT及MRI能显示环状胰腺直接征象和间接征象，6例均能显示直接征象，即胰头增大，呈环形包绕十二指肠，2例能清楚显示环状部胰管走行。间接征象包括肝内胆管扩张（2例）、胆总管扩张（4例）、胰管扩张（4例）、胰腺炎（1例）、胆囊炎（1例）。MRCP显示环状部胰管较清晰，表现为环状部胰管呈环形围绕十二指肠。结论 诊断成人型环状胰腺首选CT平扫及增强检查。 [关键词] 环状胰腺；体层摄影术，X线计算机；磁共振成像；磁共振胰胆管成像 [中图法分类号] R445；R576 [文献标志码] A CT vs MRI Diagnosis of the Annular Pancreas in the adults Li Mao,He Xiaojing,,Xie Weibo,Li Xuanfei,Zhao Jiannong(1Department of Radiology,2Department of Hepatobiliary Surgery, The Second Affiliated Hospital,Chongqing Medical University,Chongqing 400010,China) [Abstract] Objective To evaluate computed tomography(CT) or magnetic resonance imaging(MRI) findings of annular pancreas in the adults in order to improve the recognition.Methods Six adult patients with annular pancreas were retrospectively analyzed. Three cases were underwent plain and contrast-enhanced CT scanning;the other three cases were underwent plain and contrast-enhanced MRI scanning,among of them,one case was undertaken magnetic resonance cholangiopancreatography(MRCP) examination.CT and MRI findings were reviewed by two radiologists blindly,and the consensus of the final diagnosis were reached.Results Both CT and MRI findings can detect annular pancreas by visualizing not only the direct sign but also the indirect sign.The imaging findings of all cases were extension of pancreatic head which encircled the second portion of the duodenum.The indirect signs included dilation of the intrahepatic bile duct(two cases),the common bile duct(four cases) and the Wirsung duct(four cases), pancreatitis(one case) and cholecystitis(one case).MRCP may also demonstrated characteristic imaging features of annular pancreas by evaluating the relation between the biliary tree and t
