The Development of a Qualitative Model of Human Error in Aviation Systems

Abstract
A qualitative model of human errors while using aviation system will be established to identify and classify errors. Four types of human errors were identified: mistake, slip, lapses, and mode errors. An information-processing model of decision-making will be analyzed and the three stages for human decision-making: Cue Sampling, Formation of Hypotheses, and Choice Action will be discussed. The model using data from the Taiwanese Flight Operation Quality Assurance database can be used to predict errors from different decision-making stages. Therefore, this study can provide a mechanism based upon various decision-making stages and their operation levels in order to prevent aviation accidents and to enhance flight safety.

1. Introduction
Automated systems have been widely adapted in the aviation industry. The implementation of these systems increases precision and flexibility in aircraft operations. These systems are designed to lower human related accidents. Investigations have found that they only change the types of human errors that occur. They also increase the possibility of hidden problems which lead to more accidents. Rouse and Rouse (1983) claimed that human errors accounted for 60% to 90% of the accidents that occur in many of the automated systems found in aircraft, ships, or power plants. O’hare et al. (1994) estimated that 60% to 80% of accidents that occur in aviation can be explained by the same human errors. From 1980 to 1996 the U.K. Civil Aviation Authority investigated 621 aircraft related accidents and concluded that 90% were caused by human error (IATA, 1999).
