* * * * 选择性COX-2 抑制剂,首先是在外周损伤组织周围消除导致疼痛的炎症因子,在外周消炎镇痛。 同时抑制在中枢(脊髓背角、大脑)的COX-2表达上调,抑制中枢痛觉超敏,从而使患者对疼痛的感受减少。 而阿片类药物通过与阿片受体结合,仅仅阻断中枢痛觉传导,达到中枢镇痛的作用,不能抑制外周的炎症反应。因此仅使用阿片类药物进行术后疼痛管理是不全面的。如果不在源头消除产生炎性因子-前列腺素,就不能降低病人的疼痛超敏,仍会继续感到疼痛。 阿片类药物和选择性COX-2抑制剂可以互相补充,发挥协同作用。 * 250 patients who had undergone surgical procedures (in- and outpatient) in the past year completed a questionnaire about their experiences with pain before and after surgery.
